Mr. H. was a diabetic and had severe ulcers on his feet. He was a member of an HMO, and his primary care physician had prescribed a treatment regimen that was proving ineffective. In response, the primary care physician offered Mr. H. an amputation below the knee that was his only option. Mr. H. went out of plan to a local wound care center that specialized in diabetic wound treatment where he was advised that vein by-pass surgery would likely take care of his problem. The HMO denied such surgery because Mr. H. referred himself to the specialist without permission. The HMO advised Mr. H.'s family that its utilization review department was reviewing the case, but that it would take at least a month to review. Subsequently, the HMO agreed to approve such surgery, but only if done by Mr. H.'s current medical group, which did not have any physician who had ever performed vein by-pass surgery. Mr. H.'s family asked for him to be transferred to a primary care physician at the medical group that staffs the wound care center. The HMO responded that although they sometimes approve such requests, they would not do so in Mr. H.'s case and that they had already granted enough of his requests. They gave as their reason a provision in the plan documents that prevent referrals outside the plan's network when the network's physicians have the capability to perform the required procedure.

1. I am a nursing student currentantly attending an Entry-level Master's Program (which started in 2011, catered for students who have a non-Nursing Bachelor's degree but wish to pursue a career in Nursing) where I just finished my first year (1 year/out of 3 years).

"A busy medical clinic in Los Angeles is alive with activity as the waiting room seats an endless line of patients. The secured sign-in window ensured the anonymity of the patients in accordance with the HIPPA guidelines. As I sat there waiting with the patients, I found myself wondering who they were and why they were there. This curiosity turned into a deep sense of remorse as I saw the pain and discomfort in their eyes. I watched an elderly lady that was too frail to walk and appeared to be losing her battle against osteoporosis struggle to find a comfortable position. Another patient in his thirties waited anxiously as he tried to distract himself with the latest issue of WebMD. As a pharmaceutical sales consultant, I leave product samples to help the doctor better determine the efficacy of the drug as well as explain why my product should be prescribed over another. Having witnessed the misery of the patients in the waiting room, my confidence quickly faded and I discarded my glossy marketing materials and rehearsed presentation. With perspiration on my forehead, I reached out for a non-biased clinical study to help the practitioner make the best decision for his patients.

Everyday I find myself in the same situation, seated next to patients, unable to interact with them due to pharmaceutical and FDA guidelines. My position allows me to interact with medical doctors, nurse practitioners, physician assistants, and other healthcare professionals, presenting valuable clinical studies to assist the doctors in helping their patients. I am trained to understand pharmacotherapy and its clinical trials but unfortunately, this position limits my ability to directly assist the patients.

I interact with Nurse practitioners on a daily basis and I am always impressed by the tremendous impact they have in patient care. Nurse practitioners take their time in uncovering not only a treatment plan in response to a certain condition or disease, but they also partner with patients to ensure the plan is successfully followed. I experienced this as a patient when I became pregnant with my daughter. My pregnancy was very difficult and it left me bed ridden for the majority of the nine months. Since my obstetrician was extremely busy attending to deliveries on a daily basis, it was the clinic?s nurse practitioner who partnered with me to ensure that I was physically and emotionally well. This hands-on approach was very beneficial for me. It was reassuring to know that somebody was available to answer any of my questions and truly care for my mind and spirit in addition to administering procedures and dispensing medications.

At this point, I have set a distinct goal of obtaining a MSN-FNP degree to fulfill my ultimate goal of partnering with patients to provide holistic care and treatment, along with my equal interest of taking part in the latest treatment guidelines and discovery.
