HHS Initiative on Multiple Chronic Conditions Multiple chronic conditions (MCC) are significant challenges and obstacles to the health practitioners and citizens of the United States. The aim of the programs by the HHS includes prevention and management of multiple chronic conditions in the context of the United States. In the execution of its duties in accordance to the programs, HHS also offers an essential component in relation to leadership for the improvement of the health of the citizens of the United States with multiple chronic conditions.

Brief overview of the mission of the program

Multiple chronic conditions (MCC) are significant challenges and obstacles to the health practitioners and citizens of the United States. This relates to the role of the multiple chronic conditions on the health and development of the economy. In order to reduce the burden and suffering in relation to the aspect of multiple chronic conditions, the government of the United States, through its department of Health and Human Services seeks to support numerous programs. The aim of the programs by the HHS includes prevention and management of multiple chronic conditions in the context of the United States. In the execution of its duties in accordance to the programs, HHS also offers an essential component in relation to leadership for the improvement of the health of the citizens of the United States with multiple chronic conditions.

The programs by the HHS in the United States defines multiple chronic conditions as numerous conditions lasting for one or more years thus requiring critical medical attention hence limiting daily operations by individuals suffering from such conditions. These conditions include physical conditions, cognitive, and mental disorders. Some of the diseases or conditions under the management of this program by the United States' government include dementia, ongoing depression, substance addiction, HIV infections, cancer, and arthritis. It is essential to note that multiple chronic conditions refer to two or numerous conditions affecting the health condition of an individual concurrently. It is the duty of the government to enhance the health conditions of its citizens thus the notion behind adoption and implementation of various programs by the government in the management and prevention of multiple chronic conditions. HHS adopts and implements five critical concepts in the management of the multiple chronic conditions. These include

This research exercise focuses on the examination of the multiple chronic conditions: a strategic framework program implemented by the government of the United States through its department of health and human services. The program was made available in 2010 for the purposes of effective and efficient prevention and management of multiple chronic conditions in the context of the United States.

Multiple chronic conditions and management are critical to the patients, caregivers, and healthcare system thus the need for adoption and implementation of an accurate concept towards the minimization of the problem affecting the development of the nation. Multiple chronic conditions: a strategic framework tends to focus on the promotion of healthy conditions in relation to patients, caregivers, and healthcare system. This population is vital for the development of the nation thus the need to enhance their health conditions and participation towards realization of the development goals and objectives. The program focuses on the improvement of health of individuals with multiple chronic conditions across the United States regardless of age, race, or social status.

The problem of multiple chronic conditions among the citizens of the United States has escalated rapidly to the point of being a threat to the public health and medical practitioners. Multiple chronic diseases, high life expectancy, and aging of the population continue to exert maximum pressure on the future of the old population. It is essential to note that individuals with multiple chronic conditions suffer from suboptimal health conditions and subjected to high cost of healthcare with the aim of improving their health conditions. This makes it critical for the authority to increase its attention on the population with MCC in order to improve its health-care quality and costs. Another intriguing statistics is the aspect that two out of three Medicare beneficiaries have multiple chronic conditions. Individuals suffering from multiple chronic conditions are at risk of increased rate of mortality and poor daily functioning thus limiting realization of effective and efficient production.

Multiple chronic conditions affect the well-being of the society or citizens of the United States. This is because of the role of the multiple chronic conditions in limiting execution of daily activities by the individuals suffering from the conditions. Lack of an opportunity for execution of the daily activities is essential towards the realization of low productivity levels across the community. Individuals with multiple chronic conditions also suffer from huge expenses in the process of enhancing their health conditions. This makes its critical for the organization to adopt and implement an effective and efficient program with the aim of managing and preventing the development and implications of the conditions among the citizens of the United States. This program would enhance health conditions of the citizens with the multiple chronic conditions thus an opportunity to participate in the development of the nation. The program also aims at achieving societal welfare through enhancing health conditions of the society members thus reflection of a caring society.

Effect of the issue/topic on the health care system as a whole

The program focuses on the realization of effectiveness and efficiency of the healthcare systems in relation to service and product delivery to the citizens of the United States. Implementation of the program will enhance efforts of the medical practitioners and systems in the prevention and management of multiple chronic conditions. The program also facilitates reduction of the medical cost within the healthcare system thus improving the welfare within the health department. The program also offers an opportunity for the health care system to manage and prevent multiple chronic conditions through minimal resources thus effective and efficient in the improvement of the health conditions of the population.

Program goals/interventions

The program focuses on the implementation and achievement of four critical goals/interventions. The first goal of the program focuses on fostering of healthcare and public health system changes with the aim of improving health conditions of the individuals with MCC (Anand K. Parekh et al., 2011). This is because of the need for the implementation of heightened care coordination for the management of MCC in the context of the United States. In order to achieve this goal in relation to prevention and management of multiple chronic conditions in the context of the United States, it is ideal for the transformation of the delivery and provider payment systems with reference to implementation of about six objectives. The first objective relates to the identification of the evidence-supported care management models, provision of effective communication, and interactions within the healthcare system. The second objective focuses on the definition of appropriate healthcare outcomes for the population with MCC relying on maintenance of function, symptoms' palliation, and prevention of adverse use of drugs.

The third objective relates to the development of incentives and payment reforms for the purposes of management and prevention of multiple chronic conditions. The fourth objective facilitates execution of the roles of interoperable health information technology in coordinating care and preventing measures to the care providers in the context of the United States. The fifth objective focuses on the prevention of further chronic conditions through various modifications and public policies. The final objective facilitates evaluation of the models and other interventions for the purposes of effectiveness and efficiency in the service delivery. The second goal or intervention in relation to the execution of this program is maximization of the use of proven care-management and services by individuals with multiple chronic conditions. This relates to the provision of quality care and attention to the individuals with MCC thus influencing their health conditions for the purposes of enhancing their daily functioning.

The third goal in the execution of this program focuses on the provision of quality tools and information to health-care, public health, and social service workers with the aim of enhancing management and prevention of the condition in the context of the United States. The final goal or intervention of the framework or the program towards minimization of multiple chronic conditions relates to facilitation of credible research with the aim of providing adequate information for benefit of individuals with MCC.
