Health Care Delivery Models and Nursing Practice

Today, health care delivery models and nursing practice are experiencing fundamental changes in response to innovations in technologies and controlling state and federal legislation. The purpose of this paper is to examine the changes that were introduced to reform the U.S. health care delivery system in an act proposed by Senators Bill Cassidy and Tammy Baldwin in February 2022. Further, the paper also assesses the implications of these changes for the role of professional nursing leaders. In addition, an analysis concerning how quality measures and pay for performance affect patient outcomes and how these affect nursing practice and describe the expectations and responsibilities of the nursing role in these situations is followed by a discussion about professional nursing leadership and management roles that have arisen and how they are important in responding to emerging trends and in the promotion of patient safety and quality care in diverse health care settings. Finally, a description of emerging trends and two ways in which the practice of nursing and nursing roles will grow or transform within the next 5 years in response is followed by a summary of the research and significant findings in the conclusion.

The bipartisan legislation introduced by Senators Bill Cassidy and Tammy Baldwin in February 2022 titled, “the Health Data Use and Privacy Commission Act,” aims to modernize health privacy laws, particularly HIPAA, to accommodate the rapidly evolving healthcare landscape, including emerging technologies and increased generation, collection, and sharing of personal health information (PHI) (Cassidy, Baldwin Introduce Legislation to Begin Modernization of Health Privacy Laws, 2022). The legislation proposes the establishment of a health and privacy commission tasked with conducting an extensive review and offering recommendations to Congress on how to revamp existing health privacy laws. This review centers on striking a balance between safeguarding individual privacy and permitting appropriate use of PHI (Lewis, 2022).

Furthermore, nurses may face ethical dilemmas surrounding privacy, data sharing, and patient consent, especially in situations where sharing PHI is deemed beneficial for patient care. In this evolving landscape, nurses will continue to advocate for patient privacy rights while balancing the benefits of information sharing for improved patient care and outcomes, making their role vital in implementing and ensuring compliance with any new healthcare privacy laws that may result from this legislation (Lewis, 2022).

Quality measures and pay for performance programs aim to improve patient care by financially incentivizing healthcare providers to meet certain benchmarks. However, these programs can have unintended consequences if not thoughtfully designed and implemented. For example, overly prescriptive measures may lead to providers focusing narrowly on meeting numeric targets rather than taking a holistic approach to improving care. Likewise, measures that emphasize quantity over quality can result in providers placing a priority on high revenue-generating procedures versus more time-intensive care and thus costly health care team coordination. This can negatively impact outcomes, especially for complex patients requiring multidisciplinary care (Pandian et al., 2023).

As frontline caregivers, nurses play a critical role in ensuring quality measures translate to better care. They must uphold standards of ethical practice when faced with incentives to cut corners. Nurses should advocate for measures that reflect meaningful improvements, not just easily captured metrics. With their proximity to patients, nurses are well positioned to provide input on realistic goals and identify flaws in measurement design. Their responsibilities include working collaboratively with all members of the care team to implement evidence-based practices that result in sustainable gains in quality and safety. In sum, while financial incentives can be powerful motivators, nurses must help to ensure the best interests of patients remain paramount at all times.

Discussion concerning professional nursing leadership and management roles that have arisen and how they are important in responding to emerging trends and in the promotion of patient safety and quality care in diverse health care settings

Nursing leaders and managers play a vital role in responding to emerging trends and promoting patient safety and quality care across diverse healthcare settings. In their leadership capacity, nurse managers oversee the development and implementation of evidence-based policies, protocols, and best practices that guide frontline staff in delivering excellent care. They analyze safety data to identify risks and opportunities for improvement. Nurse leaders advocate for resources and system changes needed to enhance outcomes. For example, in response to the trend of increased care fragmentation, nurse leaders may spearhead efforts to improve care coordination and transitions (Marnie et al., 2020).

Effective nurse managers exhibit strong communication and relationship-building skills to foster teamwork and engagement. They provide coaching and feedback to help staff continuously strengthen clinical expertise and critical thinking. Nurse leaders create a culture of safety where nurses feel empowered to speak up about concerns without fear of retribution. Further, nursing leaders also ensure adequate staffing levels and reasonable workloads, which is foundational to safe care (Ansah-Ofei et al., 2021).

In addition, outstanding nursing leaders remain vigilant concerning emerging technologies and treatments. They provide staff with proper training and education to successfully implement new approaches that improve patient outcomes. In diverse care settings, professional nursing leaders develop policies that are responsive to the unique needs of different patient populations. Nurse managers who embrace their multifaceted leadership and management role are instrumental in delivering high-value care and advancing the nursing profession (Harrington, 2023).

Some of the more predictable and significant ways in which the practice of nursing and the nursing role will grow and transform over the next 5 years include an expanded scope of practice for advanced practice registered nurses (APRNs). Given increasing physician shortages and a growing push towards value-based care, APRNs will likely see expanded autonomy and scope of practice privileges. More states may allow APRNs to practice independently without physician oversight. This will increase access to primary and specialty care, particularly for underserved populations. Nurses practicing at the top of their education and training can help address predicted provider shortfalls.
