Though some will believe these suggestions to be inappropriate, they are imperative if HIV is ever to be eradicated. Also, if it is possible to eliminate the transfer of HIV to a child born of an HIV positive mother, then every possible action should be required. It is the requirement which will be offensive to some. However, since this study proves, beyond a doubt, that early detection greatly reduces transmission, all pregnant women should be required to undergo testing for HIV. It is only by doing so, that the safety of the baby can be ensured through the implementation of the three-arm therapy if it turns out that the mother is indeed HIV positive.

Obviously, this mandatory testing will not be effective for those mothers who do not seek any type of prenatal care. It is impossible to test them if they do not seek any type of medical assistance or care for their babies. Unfortunately, those who do not seek...

Add to that the fact that many mothers to be who are illicit drug users (another contributing factor to a higher rate of transmission of HIV) are among those who do not seek prenatal care and have low birth rate babies. This puts this group in a very high risk group. Though, the type of program may vary and it may not be all inclusive, some type of program must be developed to address these women who appear to be at the highest risk of transmitting HIV to the child. Only by devising some type of mandatory testing of pregnant women can the three-arm therapy be used to its maximum benefit.
