Question 1
Change may not be readily accepted in all instances. In some cases, change is accepted but sustenance of the same becomes a challenge later on. There are quite a number of potential barriers to change that could impede the full implementation of the suggested strategies to end nursing shortage in the OR. To begin with, failure to closely monitor change initiatives could make the desired change impossible to accomplish in the long-term. Failure to monitor the said change initiatives could lead to both deviations from the intended course and lack of commitment from those tasked with overseeing change. According to Magnaye et al. (as cited in Promes, 2016), in seeking to see through change initiatives, there is need for those responsible for the implementation of change “to scientifically track implemented change initiatives in order to accurately plan, evaluate, and monitor elements necessary for changing situation” (69). For instance, with regard to my EBP change proposal, there is need to ensure that the preparation of preceptors for their role continues within the prescribed time limits and that the preceptor program actually accomplishes the learning outcomes desired for those transiting to the perioperative nurse roles. Next, failure to align my EBP change proposal with the culture of the institution could impede change efforts after a certain period of time. In essence, this could lead to resistance to change whereby people feel that the new change effort is interfering with the generally accepted way of doing things. Towards this end, there is need to ensure that the change efforts are led by persons that not only set the tone for the change efforts, but also embody and understand the institution’s culture. My EBP change proposal, therefore, ought to be implemented in a pace that does not upset the underlying standards of conduct and values within the institution.
Question 2
There are quite a number of avenues I could make use of in seeking to disseminate my EBP project. These include, but they are not limited to, health care or nursing conferences and professional journals. The professional journal I would select for this endeavor is The American Journal of Nursing. This more so the case given that this particular journal happens to be “the oldest and most honored broad-based nursing journal in the world” (American Journal of Nursing – AJN, 2018). Towards this end, I feel that the journal has wide readership and therefore, my project would get the desired exposure. Secondly, my project falls in the category of content accepted for submissions. In essence, the journal accepts submissions from, amongst other areas, “evidence-based clinical application papers and descriptions of best clinical practices, original research and QI reports, case studies, narratives, commentaries, and other manuscripts…” (AJN, 2018). On the other hand, the nursing conference I would seek to present my project is the 28th Surgical Nursing and Nursing Education Conference (Surgical Nursing 2018) with will be held in Osaka, Japan next month (August). The conference runs from dates 8-9. According to Conference Series (2018), the theme of the said conference will be “Innovative Innovations towards Surgical Nursing and Nursing Education.” I would be interested in attending this conference to present my article due to the fact that at least two theme topics (i.e. perioperative nursing and nursing education) align with the subject matter of my project. It is also important to note that unlike would be the case with the journal submission, the conference presentation would enable me to gauge the response of the audience so as to help me define future research engagements. As Conference Series (2018) further points out, this particular conference will provide those who attend with exposure to endless possibilities in the field of nursing. My project proposes solutions to a perennial issue in preoperative nursing.
Question 3
To begin with, for a BSN-prepared RN, EBP comes in handy in the further enhancement of patient outcomes. This is more so the case given that reliance on current and most reliable clinical research could help bring down complications and effectively reduce mortality rates. Further, for a BSN-prepared RN, EBP leads to not only the development of better decision making skills, but also greater levels of confidence – both resulting in superior nursing skills. In essence, there are various ways through which I could continue to integrate evidence into my practice while at the same time further reinforcing it at my workplace. To begin with, I would continue to be inquisitive and seek the most relevant answers (backed by systematic research) to emerging clinical situations. I would ensure that I peruse through all the available evidence so as to further contribute to the advancement of nursing practice. In seeking to encourage evidence within my organization, I would encourage colleagues to form strong bonds and partnerships with other professionals in a format that would encourage systematic knowledge acquisition. Towards this end, cross boundary “teamwork needs to be strong in order to translate the evidence into improving the patient’s experience” (McSherry and Bassett, 2002, p. 99). Obstacles that could be encountered in this endeavor include lack of willingness by staff to sustain behavior change or adopt new processes, and insufficient funds to power through the best clinical evidence as supported by current research. The impact of these could be minimized in various ways. For instance, the relevance of formulating a support system as well as context for the sustenance of EBP undertakings cannot be overstated on this front. .
