Dear Mr. Matsuoka

I want to get your permission to do a research study at your business. I am getting my PhD in psychology at Saybrook University in Oakland, California. I am writing the dissertation for my research study. It is called: “The Role of Elder Care Professionals Who Speak Japanese to Japanese American Patients in Hawai’i[IS THIS APOSTROPHE INTENDED?] Eldercare”.

I want to recruit 3 to 4 patients who have worked with your staff. I need to interview them and will not reveal their names in the study. I would also like a member of your staff or to fill out an interview questionnaire. I will not reveal the identity of the person completing it. This staff member should fill out a consent form and return it to me before doing the interview.

The volunteer can complete the interview in a quiet setting at your company. The interview process should take no longer than 30 minutes. The participant will have two to three days to complete the survey. The interview results will be used for my dissertation research. The study results will not include the names of the participants. There is no charge to the Sakura House for this.
