Abstract

The objective of this study is to complete a policy analysis on examination and treatment for emergency medical conditions and women in labor. Arising from the policy analysis will be three to five options to present to the client, a representative in Congress. The holistic labor and delivery choice is in demand with many parents-to-be vying for this type of labor and delivery solution. This type of choice is one that is available in only a few medical and health care facilities in the United States. This solution saves money, time, and stress and overwork of staff with higher satisfaction among patients. It will be requir3ed that this type of health care facility be adherent to legal and regulatory requirements. Even if the medical facility were enabled within the scope of the law and legislation to send emergency labor and delivery patients to other medical faculties and even if the medical facility terminated its practice in treating labor and delivery, the reality would be that the medical facility would fail in its mission and vision which states that the medical facility intends to provide the highest level of health care to the community in which it is situated towards enabling the community to reach a higher level of health status. It is necessary that the vision of health care become broader rather than to narrow and to offer a more inclusive rather than more exclusive level of care to those in the community in which medical facilities are situated.

This study is challenging because it is necessary to identify some type of added value to the health care services provided at a medical facility that treats women in labor for emergency medical conditions while at the same time adhering to regulations and standards of treatment both legally and ethically speaking. The costs of treating patients with non-insurance are extremely high and there are regulations barring the transfer of individuals to other facilities until they have been stabilized. In the case of the women in labor treated at this facility, the span of time that the facility might need to continue treating these patients may be longer since it involves the birth of a child and means one more patient will be considered in regards to discharge or transfer of the patient. Trending now is the desire of young mothers-to-be to have choices about where and in what environment they will bear their children. These choices are very limited at present.

I. Holistic Choices in Clinical Care

Because choices for mothers-to-be are so limited and so many young women desire choices including the choice of a mid-wife rather than a medical doctor and the choice of a home-like environment rather than the sterility of the delivery room in the hospital it is proposed in this study that a viable option for the health care facility at issue is the addition of a holistic labor and delivery care program. (Overgarrd, Fenger-Gron, and Sandall, 2011, p.1) Integrated care offers the potential to do just this and moreover to offer a program that enables health care providers to assist patients in establishing better health and in maintaining that health. Integrated health is a health care ideology in which the patient's health care is managed quite simply through 'case management'. Information technology is key in this initiative in that all of the pertinent information for the patient will be linked universally throughout the healthcare domain enabling physicians and practitioners to make the best choices for the patient based on their knowledge and the latest research without potentially having adverse reactions from unintentional medication interactions. It is necessary with the large group of individuals who presently do not have health care insurance coverage that physicians and practitioners along with organizations and institutions in the health care industry approach how addressing health care needs of the population might be carried out in the most effective manner as well as do so within economic reason. Holistic health care in the labor and delivery sector of health care is a desired method of approaching labor and delivery by many women today. Overgarrd, Fenger-Gron and Sandall (2011) report "Women intending to give birth in an FMU had a significantly higher likelihood of uncomplicated, spontaneous birth with good outcomes for mother and infant compared to women intending to give birth in an OU." (p.1) According to the work of the American College of Nurse-Midwives more women today are choosing to use midwives for their child labor and delivery than ever before. It is reported that midwives "…offer evidence-based health care services. In today's world of high technology, Midwifery services provide the individualized care women need. [Doug Laube, MD, Former President, American College of Obstetricians and Gynecologists], (American College of Nurse-Midwives, 2012, p.1) It is reported as well that average costs for "…vaginal birth are approximately 50% lower than those for cesarean birth." (American College of Nurse-Midwives, 2012, p.1)) It is reported that the Office o technology Assessment analyzed nurse practitioner and nurse midwife practice at two different points in time and found that they provided medical care that was equivalent to or exceeded physician care at lower total cost." (American College of Nurse-Midwives, 2012, p.1)) Other options have been researched in this study as well including the legislation of alternative emergency treatment for women in labor and delivery and termination of labor and delivery services at the medical facility in question.

II. Three Solutions

The three solutions chosen for comparison in this study that might be utilized in generating additional revenue for the medical facility at issue are stated in the following table labeled Figure 1
