Cultural Competence:

What is cultural competence? Review how cultural competence is described in the Purnell text and the discussion comparing cultural competence to cultural sensitivity and cultural awareness. There is a good bit of discussion about these terms and how they are delineated from each other. As culturally competent advanced practice nurses, we must demonstrate sensitivity to all people groups.



There are several very well done theories of cultural competence. The Purnell Model that is in your text in chapter 2 is the one we will focus on in for our coursework in NU 611. Be sure to read about the Model in your text and start thinking about a people group in your geographical area that you interact with in your nursing practice. In the text, there are excellent descriptions of a number of cultures that are present in the United States. Purnell does an excellent job of discussing our own general culture in the US. Review the text and read what is said about your own cultural heritage. Do you agree with the discussion in the text?

1. In this course, we base our study of international management on a contingency framework. This framework allows a systematic review of all the aspects and factors related to understanding management in a global economy.
a. The framework consists of several components. Describe each component and explain how the components relate to each other.
b. Since the external environment plays such a critical role in the contingency framework, management needs to cognizant of the PESTEL components. How do these components change when a business operates internationally?

3. A critical challenge for companies that operate internationally results from differences in legal systems that countries use. Contrast the common law based system of the United States with the code based system used in most European countries. What are the important differences and their implications for international business?
4. Today, businesses operate in a global economy. What does that mean?
a. How is this global economy different from previous economic systems? Summarize our systematic comparison of the global economic system to the economic conditions that existed before the fall of the Berlin wall. Make sure to include in your summary the need for entrepreneurship and innovation and the notion of creative destruction.
b. The global economy is a free-market, capitalistic economic system that is created through the micro-level decisions that companies make. These decisions are based on the need for companies to make a profit. Explain! Consider the lecture on the reasons for companies international expansion.
5. As companies are expanding internationally they seem to go through several phases of development, resulting in different types of international companies.
a. Describe and explain the different types of international companies. What are the primary differences among them? Make sure to talk about differences in international orientation (ethnocentric etc.) and staffing strategies (PCNs etc.).
b. Give an example of a truly global/transnational company. Explain why this company fits in that category.

7. When conducting a CBRA to determine the attractiveness of a particular country, companies must also consider issues related to CSR and business ethics. In addressing the following questions, make sure to consider the Nike and Pharma cases.
a. What is CSR? How is business ethics different from CSR? Why do business ethics and CSR become an even bigger issue when companies operate internationally?
b. What is a stakeholder map and what is it used for? Give an example.
c. What guidelines for ethical behavior can we use in business? Describe how you would ensure that the employees who report to you act ethically?
d. To what extent does the Foreign Corrupt Practices Act provide guidance to companies who operate internationally?
8. What is the purpose of the Porter Diamond? In detail, describe and explain Porters four factors. Use one of the examples discussed in the lecture to demonstrate how Porters Diamond works. Using the material from the lecture on Porters model, explain why it makes sense that the World Bank provides financial aid to developing countries to support the building of infrastructure.
9. The effective functioning of the global system depends on free and open trade among countries. Protectionism, explicit or implicit, however, is still quite common and has in fact increased during the current global economic crisis. Some commentators have talked about de-globalization as a result of the global economic crisis.

So far, our focus has been on the external environment and the associated CBR. An important aspect of international business which adds a great deal of diversity and complexity involves the need for international managers to understand their people and the values they hold. These values and assumptions are shaped, primarily, by the culture these people live in. Culture represents the system of values, beliefs, norms, and actions that characterize a distinct group of people. Although there can be similarities between cultures, there are also distinct and important differences. As business "goes international", the need for sensitivity to these cultural difference and deeply held values increases.
Everything from the desire to help, respect for authority, expectations of management and control, and differing definitions of truth telling and honesty change from culture to culture. Commitment to gender equality is likely one of the most dramatically different values held in various cultures. Understanding these differences, and accounting for them, is rapidly becoming of the utmost importance.
Geert Hofstede, a scholar and researcher of culture, identified five cultural dimensions. He conducted research of over 100,000 employees of IBM in 53 countries. Hofstede identified the following dimensions:

? Power Distance addresses the equality or inequality characterizing a culture as it is reflected in approaches to management. More "power distance" implies a more autocratic system, such as in China, Indonesia, Russia. By contrast, in the US, the power distance is small, reflecting equality and "participative management".
? Individualism refers to whether individuals prefer to function as individuals (United States) or as groups (Asia).
? Masculinity versus Femininity denotes the degree to which a culture is more aggressive and competitive (masculinity) or more nurturing (femininity). This also depends on the percentage of women in management.
? Uncertainty Avoidance focuses on people's preference for structured environments versus ability to manage uncertainty. The former have a harder time dealing with change (Japan, France, Russia). The latter are more open to change and more entrepreneurial (Hong Kong, United States).
? Long-Term Orientation refers to people's emphasis on shorter-term orientations (United States, France) or longer-time frame orientation (China and Hong Kong).
Edgar Schein is a major scholar of culture. He is a professor at MIT and a consultant.
One of Dr. Schein's seminal works is entitled "Organizational Culture and Leadership".

Language tends to be a barrier of confusion and misunderstanding between cultures, and art is an easy, non-obtrusive form of communication that can help therapists exercise more improved culture-sensitive practices.

I expect to learn new, useful methods of incorporating art experientials to other cultures. I anticipate this will increase my cultural sensitivity to their specific needs, wants, and biases and of the like. This will be useful because different cultures react to the same issues differently, as different people within the same culture react to the same issues differently. Therapists are prepared for the differences between clients to be based on their exact situation in life at a certain point in time. However, they are not sensitive to the consistent underlying influence cultural traditions have on how people think, perceive, judge, act, react, as well as understand their life situation at all times. This primary, innate characteristic is often an ignored, sometimes unnoticed factor in therapy.

1. The year is 1988. Merced Community Medical Center has invited you, a cultural
anthropologist, to give a lecture to its staff. MCMC wants you to help improve its
cross-cultural sensitivity generally and its sensitivity to the Hmong in particular.
MCMC is aware of Lia's case and wants to avoid a repetition in the future. As a
frame for your lecture, you need to use Fadiman's contention that there is a
medical establishment culture, and a Hmong culture, and that what happened to
Lia was a case of cross-cultural misunderstanding. This would involve some
background on Hmong religious beliefs as they impact medical care. You also
need to put a "human" face on the Hmong (that means you should talk about Lia's
case and her parents), and provide some specific suggestions to MCMC staff to
help them be more sensitive to all Hmong patients. You need to provide some
specific suggestions as to what they can do when treating Hmong patients. What
are you going to say? (You might, among other things, discuss the "eight
questions", and why/how the outcome of Lia's case might have been different if
they had ever been asked. However, it is important that you include other
suggestions by Fadiman to the MCMC staff regarding specifically how to interact
with Hmong patients.)
2. The year is 1990. The majority of the Hmong who entered Merced in the 1980's
are still on welfare. Many Merced taxpayers are angry and are lobbying the State
legislature to change the welfare system to remove all Hmong, (or any other
immigrant who has been in the US more than 5 years) from any welfare support. In
general, relationships between Hmong and non-Hmong in Merced are becoming
increasingly volatile, as there are also protests surrounding animal sacrifice,
chanting, and other practices of shamans in Hmong religion. You are a cultural
anthropologists living in Merced, and you want to help the community to be less
ethnocentric about the Hmong, and to learn more about Hmong culture.
Specifically, you want the people of Merced to understand Hmong religion and the
reason for the various rituals. (Use the rituals involving Lia, and perhaps Lia's
case, as examples.) In addition, you need to present some of the various solutions
offered by Fadiman. You also want to persuade the people of Merced that even if
other immigrants should be cut off from welfare benefits after five years, an
exception should be made in the case of the Hmong. To do this, you will have to
explain the Hmong involvement in the Vietnam War, and why they are in the
United States at all. What are you going to say?
3. While your speech should have a concluding paragraph, I also want you to add
an additional concluding paragraph, as follows. In this final paragraph, briefly
provide your reaction to/opinion of the book. You may also indicate whether you
agree with your own "speech" or not.

Kim Gavin named artistic director for London 2012 closing ceremonies

The creative force behind Take That's acclaimed Circus tour has been joined by a costume and stage set designer for Lady Gaga and the music composer on the past five James Bond films to mastermind the London 2012 closing ceremony ??" and he spent his first day on the job criticising past efforts by Olympic hosts.
Kim Gavin, who has been announced as the ceremony's artistic director, has produced shows and performances including the record-breaking Take That tour and the band's upcoming European Progress tour, which has reunited the group with Robbie Williams. His appointment was welcomed by Take That's Gary Barlow.
"Me and the guys are thrilled that Kim has been asked to create the closing ceremony, we know with him in charge it's going to be one great celebration for the whole country," said Barlow. "We're very proud of our live shows and Kim has always been at the very heart of their creation. We wish him all the success in the world and before you ask, no, he hasn't asked us to be in the show ??" yet!"
But Gavin used his first interview to criticise ceremonies of previous years. While Beijing's show in 2008 was stylistically "amazing", Gavin said it left audiences cold, adding: "I can't remember a single tune".
The Olympic closing ceremony was the second most popular sporting event of 2008, attracting 252 million viewers across the world but Gavin said the show failed to grip those watching it live in the stadium.
"They had mass numbers of people doing the same thing and stylistically, it was faultless. But if you looked at the audience in the stadium, they were not engrossed," he said. "They were distracted with things like feeding their kids ice-cream. My objective is to make sure that doesn't happen in London: I want to engage my audience."
Gavin said the 2004 ceremony in Athens was "too traditional" and "difficult to relate to". "It was beautiful and executed fabulously but it was very local and traditional," he said. "It didn't strike a chord with me."
Sydney's 2000 event was one of the few ceremonies to garner praise but of Atlanta in 1996, he said: "I would not have said it was a great opening. I watched it, asking myself, 'Am I stylistically gripped, inspired by this show?' I had to admit that I was not."
Gavin, a trained ballet dancer who has choreographed and directed many Brit Award performances, the Concert for Diana and the Help the Hero's Concert, said he wanted the 2012 closing ceremonies to be "about celebrating where we are culturally in the UK. I want a typically British closing ceremony".
But, he added, although he will "look locally for talent", he will "also bring in performers and inspiration from outside of the UK if necessary".
"Britain has an amazing array of talent but (if) I find that I have a vision that requires something different, I will look abroad," he said. "I don't have any rules."
Gavin's comments were received calmly by the International Olympic Committee. Mark Adams, the director of communications for the IOC, said: "It would be highly unusual if a creative director didn't have very strong views on what they wanted to do, and on how it would vary from the work of others.

"Each Games organiser must produce a ceremony that works for them and the audiences they want to reach out to. We have absolute confidence that London 2012 will put on quite a show."
The London Organising Committee of the Olympic Games and Paralympic Games (Locog) said international stage and costume designer Es Devlin would be the designer.
Devlin recently worked with Lady Gaga on her 'Monsters Ball' US tour. Kim Gavin named artistic director for London 2012 closing ceremonies

The creative force behind Take That's acclaimed Circus tour has been joined by a costume and stage set designer for Lady Gaga and the music composer on the past five James Bond films to mastermind the London 2012 closing ceremony and he spent his first day on the job criticising past efforts by Olympic hosts.
Kim Gavin, who has been announced as the ceremony's artistic director, has produced shows and performances including the record-breaking Take That tour and the band's upcoming European Progress tour, which has reunited the group with Robbie Williams. His appointment was welcomed by Take That's Gary Barlow.
"Me and the guys are thrilled that Kim has been asked to create the closing ceremony, we know with him in charge it's going to be one great celebration for the whole country," said Barlow. "We're very proud of our live shows and Kim has always been at the very heart of their creation. We wish him all the success in the world and before you ask, no, he hasn't asked us to be in the show " yet!"
But Gavin used his first interview to criticise ceremonies of previous years. While Beijing's show in 2008 was stylistically "amazing", Gavin said it left audiences cold, adding: "I can't remember a single tune".
The Olympic closing ceremony was the second most popular sporting event of 2008, attracting 252 million viewers across the world but Gavin said the show failed to grip those watching it live in the stadium.
"They had mass numbers of people doing the same thing and stylistically, it was faultless. But if you looked at the audience in the stadium, they were not engrossed," he said. "They were distracted with things like feeding their kids ice-cream. My objective is to make sure that doesn't happen in London: I want to engage my audience."
Gavin said the 2004 ceremony in Athens was "too traditional" and "difficult to relate to". "It was beautiful and executed fabulously but it was very local and traditional," he said. "It didn't strike a chord with me."
Sydney's 2000 event was one of the few ceremonies to garner praise but of Atlanta in 1996, he said: "I would not have said it was a great opening. I watched it, asking myself, 'Am I stylistically gripped, inspired by this show?' I had to admit that I was not."
Gavin, a trained ballet dancer who has choreographed and directed many Brit Award performances, the Concert for Diana and the Help the Hero's Concert, said he wanted the 2012 closing ceremonies to be "about celebrating where we are culturally in the UK. I want a typically British closing ceremony".
But, he added, although he will "look locally for talent", he will "also bring in performers and inspiration from outside of the UK if necessary".
"Britain has an amazing array of talent but (if) I find that I have a vision that requires something different, I will look abroad," he said. "I don't have any rules."
Gavin's comments were received calmly by the International Olympic Committee. Mark Adams, the director of communications for the IOC, said: "It would be highly unusual if a creative director didn't have very strong views on what they wanted to do, and on how it would vary from the work of others.

"Each Games organiser must produce a ceremony that works for them and the audiences they want to reach out to. We have absolute confidence that London 2012 will put on quite a show."
The London Organising Committee of the Olympic Games and Paralympic Games (Locog) said international stage and costume designer Es Devlin would be the designer.
Devlin recently worked with Lady Gaga on her 'Monsters Ball' US tour. She has also worked with artists including Muse, Kanye West, Mika and the Pet Shop Boys as well as in opera and drama on shows for Glyndebourne, the Royal Opera House, English National Opera, the Royal Shakespeare Company, the Royal Court Theatre, the Young Vic and the Old Vic.

Health care professionals can use the Health belief model to predict behaviors along with the ecological model that incorporates the individual, people and their environment, including developmental history and culture to change behavior. According to this model, behavior is linked to the knowledge values and beliefs of a person and can be used as a way to decrease barriers. By gaining a better understanding of a culture and its values, we can influence behaviors in a positive way. This would lead to better health outcomes for the Elderly whole. Evaluate and educate elderly patients sent home from hospitals during the first 6 months of drug therapy nurses can collaborate with other healthcare professionals and act as liaisons between doctors, caregivers and pharmacists. Chapter 6: Summary Exercise The following exercise should help you to refine your abilities in culturally sensitive talking, listening, and active- listening skills.
1. With the informed consent of a friend or colleague from a cultural tradition that differs in some substantial way from your own, make a video recording 8 of a 15- minute conversation. Indicate that you are trying to practice your interviewing skills and would like to conduct an interview about his or her choice of career. Tell your partner that she or he will not have to answer any questions that evoke discomfort. Also, be sure to indicate that your professor and perhaps some of your classmates may review the recording to provide feedback about the quality of your interviewing skills. Ask your colleague for permission to both record the interview and to use it exclusively for the purpose of your learning. In other words, obtain informed consent. You might mention that if your colleague is interested in viewing the recording and perhaps providing additional feedback once you have completed the assignment, you would truly welcome and appreciate that as well.

During the interview, explore with your colleague how she or he came to make the career choice. Explore influential and motivational factors. Ask about your partner?s hopes and aspirations as well as issues and concerns regarding the chosen career. Use the skills of talking, listening, and active listening to encourage your colleague to share as much as possible about the career decision. At the conclusion of the interview, ask your colleague to reflect upon the experience and then complete a copy of the Talking and Listening Skills Rating Form contained in Appendix 11. When your partner has completed the instrument, ask her or him for immediate reactions to and feedback about the experience? perhaps by elaborating upon responses to items included in the rating form. Ask about cultural sensitivity. At what points were aspects of his or her culture especially well recognized, appreciated, or respected; when were they not? Explore nonverbal factors such as your eye contact, facial expressions, body positions and movements, physical gestures, tone of voice, rate and volume of speech as well as its modulation. Ask if you appeared genuinely interested in what she or he had to say. Ask how well your partner thinks that that you understood and ?remembered what was said, and that you fully accepted him or her? Ask about exchanges when she or he felt especially heard and understood; and those when she or he did not. Ask about your attempts to reflect back and paraphrase her words. Which times were you truly on target; when were you off? Finally, ask your partner to give you suggestions about what you might do to become more culturally sensitive and improve upon your talking and listening skills. When your partner finishes, express your gratitude, offer your thanks, and say goodbye.
2. Next, play the video recording. Prepare a word- processed transcript that accurately reflects what was said and by whom but do not use your colleague?s name. Use interviewee to refer to his or her words. Title the document ? Transcript of an Early Interview.? Identify the talking and the listening skills you used during the conversation. For example, identify as talking a statement you made or a question you asked that came from your frame of reference. Identify as active listening your attempts to communicate your un-derstanding of your partner?s expressions. Use your word- processing software to organize the transcript according to the format outlined in Table 6.1, but be sure to disguise the identity of the person you interviewed.

After you have prepared the transcript, use it along with the video itself; your ?colleague?s responses to the Talking and Listening Skills Rating Form and her or his comments and suggestions; and your own observations to evaluate your ?proficiency in the talking, listening, and active- listening skills. Word- process a two- to three- page (500? 750 words) report titled, ?Evaluation of an Early Interview.? In your paper, ?respond to the dimensions suggested by statements in the Talking and Listening Skills Rating Form by addressing questions such as the following: How did your verbal and non-verbal communications reflect sensitivity and respect for various aspects of your colleague?s culture? How did your facial expressions, head movements, tone of voice, body positions, and physical gestures communicate that you were genuinely interested in your partner and everything he or she said? How clearly and audibly did you speak, and what might your tone of voice and speech modulation suggest about your attitude toward your colleague? Were your words and language familiar to your colleague so that she or he could easily understand everything you said? How did your verbal communications and nonverbal expressions convey nonjudgmental interest in, attention to, and respect for your partner and everything he or she expressed? How well did you remember what was expressed during the interview?
How did you summarize and reflect back your partner?s messages so that she or he felt truly heard and understood? Conclude the report by identifying those aspects of the talking and listening skills that you need to practice more and improve. Outline the dimensions of your plans to do so. If you have your interview partner?s permission and informed consent, ask one or two classmates or an instructor from the school or department of social work to review the videotape and offer evaluative feedback about your expression of cultural sensitivity and the nature and quality of your talking, listening, and active- listening skills. ?Consider the feedback. If constructive, revise your word- processed report to accommodate the new information. Once you have edited and finalized your report, include it and the transcript in your Social Work Skills Learning Portfolio.
2. Finally, reflect on the content and the exercises contained in this chapter; and your recently completed interview evaluation report. Based on your reflections, word process a succinct one- to- two page letter to yourself. Prepare it as a formal business letter. Address it to ?yourself along with a note to read it again one year from now. In the letter, describe a plan by which you might become a more culturally sensitive and more empathic listener who can communicate with people in a warm, genuine, and respectful manner. Identify how you would know that you had progressed in these areas. When you have finished, title the file ? One- Year Letter to Myself ? and include the letter in your Social Work Skills Learning Portfolio. Make a note in your calendar to read the letter in one year?s time.
