APRNs and State Regulation

Introduction

The evolution of healthcare practice has led to the development of advanced roles such as the Advanced Practice Registered Nurse (APRN). Specifically, the role of the Psychiatric Nurse Practitioner (PNP) has been integral in addressing the significant gaps in mental health care. This paper presents my personal philosophy and practice framework, which aligns with the work of nurse theorist Hildegard Peplau. This paper also identifies the educational, certification, and licensing requirements for APRNs in Florida, and it discusses the implications of prescriptive privileges for APRNs in terms of patient safety and care. The distinctions between prescriptive authority, credentialing, and clinical privileges, and their impact on patient care are also described. Lastly, the development of the APRN role is evaluated from a global perspective.

Personal Philosophy and Framework

As a PNP, my philosophy is centered on holistic and patient-centered care, focusing on a bio-psycho-social-spiritual model. I believe that this approach allows me to provide comprehensive and culturally sensitive care, focusing on a patient's entire well-being rather than only their symptoms. I am committed to evidence-based practice, lifelong learning, and the integration of current research into my patient care strategies. Furthermore, I acknowledge the competencies set forth by the American Nurses Association (ANA) and the American Psychiatric Nurses Association (APNA), which includes standards for psychiatric-mental health nursing, practice across the lifespan, and primary care.

The role of the PNP is multi-dimensional, involving diagnosing and treating mental health conditions, counseling, educating patients, and often coordinating care across a multidisciplinary team. I am guided by the principles of autonomy, beneficence, non-maleficence, and justice. If I can continuously evolve and adapt to meet the needs of all my diverse patients, I can empower my patients in a culturally sensitive way, which in turn will enhance their abilities to manage their mental health.

Nurse Theorist

The theorist I align my practice with is Hildegard Peplau, whose Interpersonal Relations Theory is especially relevant to psychiatric nursing. Peplau emphasized the importance of the nurse-patient relationship and viewed nursing as a therapeutic, interpersonal, and collaborative process (Moreno-Poyato et al., 2021; Pratt et al., 2021). This model provides the foundation for my philosophy as it promotes empathy, communication, and understanding, essential aspects when dealing with psychiatric patients (Granados-Gamez et al., 2022). It is a model that helps trust to bloom between the nurse and the patient.

Educational Courses and Professional Requirements

In Florida, to become an APRN, one needs to hold a valid Registered Nurse (RN) license, earn a Master's degree in a specialized field of practice (such as psychiatric nursing, the field in which I am working), and pass a national certification examination (Florida Board of Nursing, 2023).

Educational courses usually include advanced pathophysiology, advanced pharmacology, advanced health assessment, primary care, and specialty psychiatric-mental health courses. Clinical hours, often exceeding 500, are also a significant requirement (AACN, 2023). In many countries, APRNs have taken on a more prominent role in primary healthcare, chronic disease management, and health promotion and prevention. This evolution is consistent across countries with different health systems, indicating a global trend towards increased utilization and recognition of APRNs.

The scope of practice, regulatory frameworks, and educational requirements for APRNs vary from country to country. Yet, there is a general trend towards expanding the role of APRNs, increasing their autonomy, and recognizing their critical contribution to healthcare (Thompson & McNamara, 2022). The World Health Organization has also emphasized the role of advanced practice nurses in achieving universal health coverage.

Conclusion

Healthcare delivery calls for advanced practitioners like PNPs who are equipped with the knowledge, skills, and competencies to meet patients' diverse needs. Florida's educational, certification, and licensing requirements ensure that competent APRNs are ready and able to serve the public. The grant of prescriptive privileges, credentialing, and clinical privileges have enhanced the APRN's role, ultimately promoting patient safety and improved health outcomes by increasing access to competent care.
