At least two systems appeared to be in place in the family medical center that impacted differences in the treatment of patients in this context. One system is formal and intentional: military rank and the deference it affords. The other system is informal and unintentional (at least from a policy problem perspective): discretion granted to street-level bureaucrats in the performance of their day-to-day duties and responsibilities. This research informs the literature on policy implementation and sociology, particularly that related to social class and status.
