Health literacy plays a huge role in the outcome of many patients today. The number of people returning to their primary care providers and emergency room due to the fact that they did not understand discharge instruction on how to manager their own care or take medications properly is over whelming and increasing at an alarming rate due to poor health care literacy. (The Health and Human Department of Health, 2011). defines Health literacy as the capacity to obtain, process, and understand basic health information and services needed to make appropriate health decisions. Nearly 9 out of 10 adults have difficulty using the everyday health information that is routinely available in health care facilities, retail outlets, media, and communities. Limited health literacy is associated with poorer health outcomes and higher health care costs (Department of Health and Human Services, 2011). Health literacy includes the ability to understand instructions on prescription drug bottles, appointment slips, medical education brochures, doctor's directions and consent forms, and the ability to negotiate complex health care systems. Health literacy is not simply the ability to read. It requires a complex group of reading, listening, analytical, and decision-making skills, and the ability to apply these skills to health situations (National Network of Libraries, 2011). The purpose of this research utilization proposal is to evaluate how health care workers put to use the methods demonstrated and assess the outcomes through follow up phones calls to recent patients to obtain feedback on their understanding of their discharge instructions received and if the communication was satisfactory. This will help determine if health care workers are utilizing the provided methods to help increase health literacy and decrease the amount of patients returning to their primary care and emergency. According to Healthy People 2020 (2011) effective use of communication and technology by health care and public health professionals can bring about an age of patient- and public-centered health information and services. A proposed solution would be to incorporate a continuous training class to include all members of the health care team that are directly involved with patients starting from the receptionist to the discharge team. The program will include methods and the use of plain language, free of medical jargon sitting face-to-face with the patient, use of simple diagrams or pictograms to illustrate explanations and the use of educational materials geared to low health literacy individuals. A review of publications from The Agency of Health Research and Quality, Institute of Medicine, Healthy People 2020 as well as various research article supports need to implement a continuous training course to address health literacy. Background
Essay should reflect knowledge of current clinical priorities in healthcare for Veterans and it will use academic program to impact quality of nursing care provided to Veterans.

Academic program is DNP in Executive Nursing Leadership. My medical center recently designated as a 1A facility, which means all services must be provided for evening and weekend hours. We are in process of ensuring mental health services are available during these hours . I will work the evening hours of 12:30 to 11:00 p.m. This tour will include working every other weekend. Change management skills and strategic planning are essential components that will be required to ensure a smooth transition. Marketing and effective collaboration will also be required. I am currently employed as Program Coordinator for Mental Health Intensive Case Management (MHICM) Programs, which is an Executive Career Field (ECF) position. ECF employees responsible to ensure the T21 initiatives are successfully implemented.

1. Describe the current specific priorities for clinical care for veterans and nursings role in your facility in providing healthcare to meet those clinical needs. Examples of clinical priorities include but are not limited to mental health, traumatic brain injury, homelessness, PTSD, virtual care, diabetes care, rehabilitation, long term care, womens health, and rural health.


Providing mental health services in the home, in the community, and rural areas. Close liaison with homeless program staff, primary care clinics to assist with meeting goals for T21 Initiatives, which includes missed opportunities, clinical video telehealth (CVT), and no shows.

2. Describe your role in the following key issues for Veterans:

A. Access to care

Liaison for Veterans who present to ER for mental health related issues for evening hours until 11:00 p.m.

B. Veteran centered care

Assist with providing alternatives to inpatient care for mental health related issues
CVT available for evening and weekend hours
Patient preference for providers and appointments

C. Performance Measures
Ensure screenings are completed at encounters to ensure that ECF Measures are met. Includes Suicide Risk, Depression, PTSD, Tobacco Use, Alcohol Use, and HUD-VASH Vouchers if homeless

D. Transition Care (DOD to VA).
Schedule for appropriate providers, clinics, and follow-up

Examination and Treatment for Emergency Medical Conditions and Women in Labor

You are a consultant specializing in policy analyst. Based on the Examination and Treatment for Emergency Medical Conditions and Women in Labor legislation above you will complete a policy analysis with 3-5 options for a client, Representative Moody, to consider.

Representative Moody is in Congress and up for re election in the next year. She seeks another term in office. Representative Moody?s constituency consists of three private regional medical centers with a trauma unit that receives referrals from 5 or more small facilities nearby with minimal trauma status for their Emergency Department.

In the past, 2 of the 3 medical centers supported Representative Moody with campaign funds at the allowable limits. Her constituency has many uninsured citizens due to high unemployment in the area. She is vocal about the need for health reform and the health care to all citizens. She also adheres to fiscal restraint and the absence of bail outs for private facilities. She knows that Medicaid dollars and unemployment dollars to support the unemployed and uninsured is at a minimal for the state. Also, the largest health care corporation that owns two of the three medical centers in his/her catchment area is threatening to refuse Medicare and Medicaid patients to ensure survival in the bad economy. An increase in unfunded mandates for urgent care may push the corporation into private payer only. There are very affluent families in the area that could support the health care services offered by two facilities catering to designer medicine and concierge services.

be consistent with the Representative Moody?s values and
address the issue identified in the problem statement
You will:

Identify 3-5 options
Identify criteria that will be used to evaluate the options
Identify pros and cons for each option.
Use a side-by-side table (MS Excel would be appropriate for this) to assist in analyzing the options.

Description: Develop a 1,750 to 2,100-word lesson plan for an education program that is intended for a particular target audience.

Case Study IV - Envisioning the Future
Instructions: The final assignment for our course in physician practice management builds on the readings this week. Working from the readings and other relevant material. This assignment asks you to place yourself in the role of physician leader of a medical group practice. You may continue to use the example of the group presented in the last two case studies, or describe another group of your choosing. The assignment is to prepare a memo for your fellow group practice physicians that articulates a vision of what the near future will bring for the medical group practice in terms of the organization and the environment the group will be facing. The memo builds on this vision to argue a direction for the strategic planning of the group moving forward.
Case Study IV ? Envisioning the Future
The final assignment for Physician Practice Management is your opportunity to think about the future of medical group practice. The readings for this week (along with last week?s hospital-physician relations article) present contrasting scenarios of what might become the dominant organizational model for the practice of medicine. Coupled with the incentives and projects included in the Patient Protection and Affordable Care Act (H.R. 3590) that was signed into law by President Obama on March 23, 2010, it is clear that there will be substantial changes in the organization of medical care in the near future in the United States.
However, less clear is what those changes might be, and what model of organization for medical groups will be dominant in the future. There are numerous frameworks of organization currently in the field, including single-specialty groups, multi-specialty groups, academic medical centers, community health care centers, risk-bearing comprehensive care organizations (such as Kaiser-Permanente), and government-based medical organizations such as the VA system or the Indian Health Service.
And your vision what the future of medical group practice will (or should) resemble can depend in large part on your perspective of the health care industry. Health policy specialists write and research regarding organizational structures such as the Patient Centered Medical Home and/or the creation of Accountable Care Organizations. Patients may view the former as reminiscent of the gate-keeper HMO models that dominated the early 1990?s, and perhaps the latter as government-inspired creations on the passage to a national health system. The insurance industry could be seen as supporting large regionally integrated systems that compete on efficiency and outcome as a method to reduce premium costs. Or alternately, they could favor the highly fragmented, cottage-industry status that currently exists as a means of retaining maximum leverage in the negotiation of covered services and payment rates.

As you begin to move into (or further into) positions of management leadership in the health care industry, being able to shape and articulate a future state will be a key component of your skill set. For this assignment, you should use the literature available (and other relevant material that you have reviewed) to draft a memo for a medical group practice in which you are a physician. Your role in the organization is as the physician leader, either Chair of a Board of Directors, managing partner, or similar position. Your memo is being developed to provide a vision of the strategic direction the group should pursue, based on your presentation of the anticipated future of medical group practice. For example, if you are a single specialty surgery group in a major metropolitan area, and the information you review convinces you that accountable care organizations are the future of health care in your region, then you might present a case for a strategic alignment with (or acquisition by) the dominant integrated health system in your area.

The focus of your memo should be on the argument you create regarding the future you envision for medical group practice medicine in your region, and based on the nature of the your organization, what should be the plan going forward. The assignment clearly makes the assumption that there will not be (in the foreseeable future) one model for the delivery of care in the United States. (Not even Dr. Donald Berwick could hope to achieve that in his wildest dreams). But by the same token, the organized delivery of care will not maintain a status quo. The pressure of changes in our mechanisms of delivery insures that new organizational structures and alignments will evolve. The challenge is to determine what will occur on a local or regional level, and position the organization to continue to achieve its mission. This is healthcare management leadership.
One final note for this assignment: As you are probably even more aware than I, your physician group practice collogues are very busy, and therefore your memo should be succinct and pertinent to both capture their attention and make your case convincingly. This assignment should be completed in a maximum of five pages, not including any references or endnotes.
