LETTER OF INTENT
I write this letter of intent to the DDA with the objective of enrolling as a HCBS Medicaid Waiver Provider operating within the ............................ area. The company is committed towards ensuring the best outcomes among persons diagnosed with various developmental and intellectual disabilities. Amongst other things, the company seeks to facilitate the successful integration of such persons back into the community. More specifically, the company is committed towards ensuring that persons diagnosed with developmental and intellectual disabilities are provided with the relevant support and equipped with the capabilities to take charge of their own lives and become productive members of the communities or societies in which they live. Towards this end, the range of services offered on this front are not only personalized, but also optimized to address the unique needs of those diagnosed with various developmental and intellectual disabilities.
My name is XXXX XXXX. I’ve been an RN for a little over 15 years. During this period, I’ve acquired a wide range of skills and capabilities on multiple fronts including, but not limited to; patient care, critical thinking, and team-work. A significant portion of my career has been spent in the ER where I worked as part of a wider team in the provision of emergency medical care and assessment of diverse patient needs. In the most recent 5 years, I have been working in mental health facilities – specifically as a psychiatric mental health practitioner. It would be prudent to note that mental health happens to be part and parcel of the overall wellbeing. Towards this end, my role as a psychiatric mental health practitioner essentially transcends all healthcare facets. In the said role, I am responsible for not only the assessment, but also the diagnosis as well as treatment (medication prescription and provision of therapy) of the various mental health conditions and substance abuse problems that patients present with. As a mental health provider, I am experienced in the provision of the relevant assistance to persons, families, and entire populations via public health policy, program development and administration, practice, research and public health education.
Some of the services that the aforementioned company intends to provide have been listed below:
In-Home Support Services: These services will be availed to those diagnosed with developmental/intellectual disabilities and live at home (at their natural home or with a family member). The company will train and equip Direct Support Professionals who avail the relevant support or assistance on an hourly basis (8 hours max) to such individuals, i.e. in relation to the further advancement of daily living skills.
Respite Hourly: This is a service available, on a short-term basis to individuals who present with various developmental/intellectual disabilities, are enrolled in the Waiver services and live with a member of their family. In essence, the said services come in handy when a caregiver or family member with whom the person with developmental/intellectual disabilities lives falls sick or travels. The company will avail trained personnel to step in, in such scenarios, and provide the relevant support to the person with developmental/intellectual disabilities – either in a company facility or in their natural home.
Individualized Day Support (IDS): This particular service is firmly rooted in the community and is designed to equip persons with developmental/intellectual disabilities with various relationship building and interpersonal skills and capabilities. The services are individualized in the sense that they are tailored to address the specific needs of the person with developmental/intellectual disabilities. The company has personnel with the relevant Person-Centered Thinking Philosophy orientation that are capable of directing efforts towards the provision of individualized day support to persons having developmental/intellectual disabilities.
Companion Services: The company will avail the assistance (non-medical) needed to ensure that persons with developmental/intellectual disabilities attain personal goals and satisfy individual needs, i.e. as highlighted in their Individual Support Plan. Personnel responsible for the provision of this support will be equipped with the skills and capabilities required to embrace the need for people to take charge of their own lives on the basis of their wishes and desires, and develop the rights systems and structures to advance this need.
