Your Term Plan: On the page labelled YOUR Term Plan, indicate your term goal(s), stretch goal(s), and the tasks you will accomplish during the weeks of the term. Your tasks should make clear what you will be working on and your plans for appropriate substantive submissions to your Faculty Advisor (refer to the syllabus for more information). Remember that you must demonstrate substantial academic PROGRESS each term to receive a satisfactory grade for the course. Students must complete a minimum of Phase 1 to obtain a satisfactory grade in during the first enrollment of the project course. Following the successful completion of Phase 1 in the next enrollment of the project course, students must complete a minimum of Phase 2 to obtain a satisfactory grade in the project course.

Begin the process of identifying change champions among the staff to help in implementing the project. I plan to also hold sessions with the change champions to communicate the finalized implementation plan and clarify their roles in the project. On day 7, I plan to hold a physical session with all the clinical staff to communicate the implementation plan, which will officially mark the beginning of the project implementation phase. I plan to submit my time log and journal for weeks 1 to 3 by the end of day 7 of week 3

I plan to hold a session with all the clinical staff as well as the project mentor to share the finalized communications plan by day 7 of week 3

I also plan to have completed the administration of the knowledge-assessment questionnaire and received all completed questionnaires back for submission to the trainers for marking/grading by end of day 7. The grade received before the training will serve as baseline data and will be compared to the grade received after the training to assess the project’s effectiveness in growing knowledge on medication safety.

Begin training sessions with the six-hour session (two hours daily for three days) on medication administration

Your Notes: The training sessions entailed a significant amount of group work. This drew my attention to the concept of interdisciplinary partnerships for effective collaboration. I conducted a literature review and completed a concept paper on the strategies that I intend to use to foster interdisciplinary collaboration when implementing the project. The week 6 time log and journal were submitted at the end of day 7, detailing the challenges faced and strategies used to address them.

Complete the trainings on dose calculations and medication sensitivity. As part of assessing progress, I plan to hold a brief evaluation exercise with the staff at the end of the third topic to obtain their views on the training

Your Notes: The training on how to manage medication errors attracted a lot of views from the staff and was not completed as scheduled. Complete the training on international patient safety guidelines, which is the last topic to be covered in the project education program.
