Not only do primary care providers recommend wearing sunscreen with an SPF above 15 — magazines and popular media reinforce this advice as well. However, according to Planta (2011), evidence-based research does not support the idea that sunscreen protects against cutaneous malignant melanoma (CMM). In fact, although rates of sunscreen use have increased in recent years, rates of this particularly deadly form of skin cancer have continued to rise at approximately 3% per year (Planta, 2011).

There is also a correlation between sunblock use and a greater likelihood of developing sunburns, which itself elevates the risk of melanoma. Even children whose parents applied sunblock more regularly showed higher rather than lower rates of skin cancer, according to one Swedish study of children aged 5–7. The U.S. Environmental Protection Agency has concluded that "there is no evidence that sunscreens protect you from malignant melanoma" (Planta, 2011).

Epidemiological Paradoxes in CMM Distribution

Studies are contradictory regarding sunscreen efficacy in preventing skin cancer of all varieties. Some evidence suggests that sunblock may reduce the risk of squamous cell carcinoma but not basal cell carcinoma. More importantly, while there is convincing evidence that non-melanoma skin cancers are related to cumulative sun exposure, the same association with CMM is far less established. If CMM were related to cumulative sun exposure, outdoor workers would be expected to have a greater incidence than indoor workers — yet that is not the case (Planta, 2011).

In fact, CMM is increasing most rapidly among individuals who spend the majority of their time indoors and who receive three to nine times less solar UV radiation than those who work or recreate outdoors. As Planta (2011) notes, "there is a higher incidence of CMM among whites living in northern states such as Delaware, Vermont, and New Hampshire (>30 per 100,000), which enjoy less year-round sunlight and UV radiation than southern states such as Texas, Florida, Arizona, and New Mexico (<25 per 100,000). In California, whites living in San Francisco had a CMM incidence of 30.5 per 100,000, whereas those living in Los Angeles had an incidence of 24.9 per 100,000." These geographic patterns challenge the straightforward assumption that more sun equals more melanoma risk.

UVA, UVB, and the Limits of SPF

The rationale behind recommending sunblock rests on the fact that ultraviolet (UV) radiation is a recognized carcinogen, and sunblock is designed to filter it. Both UVA and UVB radiation are emitted by the sun, but because UVB was historically believed to be the primary cause of all skin cancers through direct DNA damage, early sunscreens were developed to block UVB with little or no protection against UVA. Critically, the sun protection factor (SPF) rating on sunscreen packaging refers exclusively to protection against UVB radiation and says nothing about UVA protection (Planta, 2011).

Recent evidence indicates that UVA also plays a significant role in melanoma development through "oxidative stress, free radical generation, and the degradation of vitamin D" (Planta, 2011). This is supported by elevated skin cancer rates among tanning bed users, even though artificial tanning light emits a higher ratio of UVA to UVB than natural sunlight. Some sunscreen ingredients — including benzophenones, titanium dioxide, and zinc oxide — offer greater UVA protection than conventional formulations. However, even the most effective UVA blocker, titanium dioxide, filters only 25% of UVA radiation (Planta, 2011), leaving a substantial gap in protection.
