We all know the importance of healthy eating and how it can affect our health. Fast food is one way which can affect our health in a bad way. Excessive fast food intake results in obesity and thus opens the gateway to many or health problems. For years people have been waging a strong battle against large multinational fast food, partly complicit in these results.
A recent study has once again made clear the negative effect of junk food on the health and its inevitable effect on weight gain and obesity. It has been found that fast foods (soft drinks, burgers, pizzas, hot dogs, chips etc) are not only unhealthy, but replace the diet with other foods such as fish, fruit, cereals , vegetables and dairy products. Fast food contains saturated fats, lack of fiber, and abundance of certain sugars which attracts young people and favors the accumulation of fat.

In this paper, we present an Individual clients health history examination on the basis of Gordon's Functional Health Pattern Assessment.
1. Perform a health history on an older adult. Students who do not work in an acute setting may "practice" these skills with a patient, community member, neighbor, friend, colleague, or loved one. (If an older individual is not available, you may choose a younger individual).
2. Complete a physical examination of the client using the "Individual Health History and Examination Assignment" resource. Use "Functional Health Pattern Assessment" as a guideline to assist you in completing the template.
3. Document findings of complete physical examination in Situation-Background-Assessment-Recommendation Format(SBAR) by using the SBAR format. Refer to the "SBAR Template," located on the National Nurse Leadership Council website at http://www.ihs.gov/medicalprograms/nnlc/documents/SBARTEMPLATE.pdf as a guide. Document the findings of the physical examination in a Word document.
4. Using the "Individual Health History and Examination Assignment," provide the physical examination findings summary with planned interventions for the client. Include any community services in the interventions.
Health Belief Model in Nursing: Theory and Application
During the 1950's, the Health Belief model (HBM) was developed from the field of social psychology. The theoretical framework offers an explanation of why individuals are motivated to participate in preventive health behaviors. The model has five perception constructs of susceptibility, severity, benefits, barriers, and cues to action. In this setting the HBM predicts what prevention behaviors diabetic patients will engage in to avoid foot pathology and ultimately amputation. Current research indicates that the Health Belief Model (HBM) is the most common model used to study health- related behaviors. According to Ganz, Rimer, and Lewis (2002) an assumption of this model indicates people are more inclined to demonstrate disease prevention activities when they perceive (a) an increased susceptibility to the illness; (b) the illness is severe; (c) the actions are valuable; (d) the behavior has few obstacles; and (e) are prompted to execute the actions.
