This case analysis is concerning a 64 year-old woman named Freda Smith referred to a local hospital in the community health center for ongoing care. Freda Smith was discharged home two days ago and diagnosed with rheumatoid arthritis ten years ago and chronic heart failure five years ago. Freda has a cataract in her left eye which will be removed in three months. During the hospital stay Freda was diagnosed with Type 2 Diabetes and commenced on insulin, her regular BGL have been ranging from 18.5 to 25.6 mmol/L. Freda lives with her husband John, in a two-bedroom second storey unit. This is a lift access within the building. John works full time as a builder and Freda works in his office as the office manager. John and Fred have four children, three sons and 1 daughter all of them live overseas. Freda is170 cm tall and weighs 110kigs.
