¶ … afternoon of the 14th of September 2005, I left my home at (add address) by car to visit a friend. I was on my way to the location when I observed the following incident. While stationary at a traffic light I saw a vehicle, a blue sedan, go through the red stop light and enter into the flow of oncoming traffic. I could not see any cause this action and it seemed to me that the driver had simply not seen the red light. He was not traveling at an excessive speed. This resulted in an immediate collision with an oncoming vehicle, a red Lexus. This was followed by a series of subsequent collisions with a number of vehicles behind the car which had been hit by the blue sedan. In total I counted five vehicles that were involved in this accident.

I then observed that the driver of the first vehicle, the blue sedan, which had initiated the accident, seemed to be unconscious or injured and was slumped over his steering wheel. Smoke was being emitted for this vehicle, which suggested the possibility of fire. The driver of the Lexus was also injured with a slight limp, but was able to walk. He in fact emerged from his vehicle and went over to the blue sedan where the driver was not moving. He and two other people helped the unconscious driver from the first vehicle and carried him to the sidewalk. I also saw that at least three of the people involved in the secondary accident also seemed to have slight injuries and one man had a cut on his head which was bleeding severely. There was a period of confusion of between five and seven minutes before a police car and an ambulance arrived on the scene. At this point I also observed that the driver of the blue sedan had regained consciousness and was able to walk to the waiting ambulance. His car was badly damaged but there was no evidence of fire. I then left the scene of the accident as the traffic authorities required the area to be cleared for further investigation.
