Psychiatry: Individual Counseling- Conceptualization Presentation

A patient suffering from depression required individualized counseling intervention, for which Cognitive Behavioral Therapy (CBT) was deemed suitable. The symptoms of his depression were evident in the form of low mood, feelings of hopelessness, feeling annoyed easily, loss of motivation, finding it challenging to make the right decisions, and even sometimes being low on self-esteem. The patient lost his wife, whom he had married ten years ago. It was his love marriage, and the grief was extreme for him. He seemed to be experiencing these emotions for the past four. He also has two daughters and wants to be normal in life routine tasks, especially for his children.

For this patient, CBT intervention was believed to be effective since it would help in altering his behavior and attitudes towards life and the people around him. As his mood swings are costing his employment, it is advised that CBT intervention with pharmaco-therapy should be conducted. This combination is explored as the most effective intervention strategy for patients suffering from depressive disorders as their relapse rate is quite low with this treatment (Gautam et al., 2020).

Some useful CBT techniques that could be grouped for formulating an individualized CBT therapy are cognitive restructuring, exposure therapy, and relaxation or stress reduction exercises (Pietrangelo, 2019). The cognitive restructuring would help in enabling the patient to stop him from over-generalizing that if one bad thing happened at one time, it would again. Placing too much emphasis on small details and complicating the situation would help him reduce his stress. He needs to have productive thoughts after his wife’s death so that positivity could reflect in his thinking and behavior for bringing betterment in his home and job life. Exposure therapy would also work the same way as he would be trained to face his fears. He fears that he is nothing without his beloved wife and cannot lead his life without her. His self-esteem is low, thinking that he cannot manage his two daughters independently, which needs to be changed altogether. Moreover, relaxation or stress reduction exercises like deep breathing exercises and muscle relaxation activities would help him lessen his stress and anxiety for gaining a better sense of control.
