Most of Jose's problems can be subsumed under two interrelated classifications. The first class of Jose's problems pertains to his psychological issues, which stem from his schizoaffective disorder. A lack of connection with other individuals fosters his inability to eat properly, to use drugs, and to withdraw from group activities. The second group of problems is his physical ailments, stemming from his diabetes. Being overweight contributes to worsening his asthma and sleep difficulties. The diabetes contributes to his other physical problems, which increase Jose's isolation from others and impede his ability to engage in acts of basic self-care.

Create a treatment plan that will address one of the issues identified in the above case study.

At present, Jose is unable to engage in routine self-care, so he must continue to remain in his current environment, which is designed to give him additional assistance to address his incontinence and other physical issues. But a program must be established to ensure that Jose is eventually able to re-enter society and become somewhat functional. Social isolation is one of the most debilitating symptoms of schizoaffective disorders. Meeting with a nutritionist to get his diabetes under control with proper food intake is an important first step, and a nutritionist can work with a substance abuse counselor to treat Jose's addiction to sweets and drugs in tandem. These counselors will also be able to establish a sense of intimacy with Jose to decrease his social isolation.

Although Jose will still need physical care, if his diabetes can be brought under control along with his drug dependence, the impact of his physical issues will be mitigated and also he will be less likely to socially withdraw from supportive group activities. Continuing to meet with a nutritionist and substance abuse counselor on a regular basis after his release is important to keep Jose socially connected, and to ensure he maintains group activities and proper self-care.
