Severity and prognosis depend on the amount consumed, the pattern of drinking, the duration of consumption, the presence of liver inflammation, diet, nutritional status, and genetic predisposition. While steatosis is virtually benign, morbidity and mortality are both high in liver cirrhosis. Survival rates for advanced cirrhosis are limited to one to two years, and patients with severe acute alcoholic hepatitis face up to a 50% mortality risk (NCBI, 2014). Long-term intake of more than 30 grams of absolute alcohol per day raises the risk of alcoholic liver disease (ALD). Liver disease is almost certain to develop from long-term consumption of more than 80 grams of absolute alcohol per day (Bruha et al., 2009).
Alcoholic liver disease (ALD), also known as alcohol-related liver disease (ARLD), is damage to the liver caused by alcohol misuse (NCBI, 2014). Symptoms typically do not manifest until the liver has been seriously damaged. The liver is the most complex organ in the body after the brain. It filters toxins from the blood, helps digest food, regulates blood sugar and cholesterol levels, and helps fight infection and disease. It is highly resilient and capable of regenerating itself. However, every episode of alcohol consumption destroys some liver cells. While the liver can produce new cells to replace those that die, prolonged alcohol use over a number of years reduces its regenerative capacity and eventually damages it (NCBI, 2014).
Stages of Alcoholic Liver Disease
The three stages of ARLD or ALD are alcoholic fatty liver disease, alcoholic hepatitis, and cirrhosis (NCBI, 2014; Bruha et al., 2009). Fatty liver disease can result from heavy drinking over even a few days. It is reversible, and liver health can be restored if drinking is stopped for two weeks. Alcoholic hepatitis results from continued alcohol overconsumption, which inflames the liver. Liver health can be restored if drinking is permanently stopped; otherwise, it becomes a life-threatening illness. Cirrhosis is the final stage, in which the liver is substantially scarred. It is generally irreversible, but immediate cessation of drinking can slow further damage and significantly increase life expectancy. Without cessation, life expectancy is limited, with a 50% survival rate of approximately five years. Complications are likely and life-threatening, including internal bleeding, increased toxins in the brain (encephalopathy), fluid accumulation in the abdomen (ascites) linked to kidney failure, and liver cancer (NCBI, 2014; Bruha et al., 2009).
Signs and Symptoms
ALD and ARLD produce a range of conditions and associated symptoms (NHS, 2013). Symptoms do not typically appear until the liver has been seriously damaged. Early symptoms include malaise, weight loss, loss of appetite, jaundice (yellowing of the eyes and skin), swelling of the ankles and abdomen, drowsiness or confusion, vomiting of blood or blood in the stools, and diarrhea. Advanced symptoms develop as the liver becomes more severely damaged. These include jaundice; edema or swelling of the extremities due to fluid buildup; ascites; severe skin itching; hair loss; clubbed fingers; blotchy red palms; considerable weight loss; muscle wasting; weakness; confusion and memory disruption; insomnia; personality changes due to toxin buildup in the brain; vomiting of blood; black stools caused by internal bleeding; frequent bruising and bleeding; and increased sensitivity to alcohol and drugs. This last symptom results from the liver's failure to process alcohol and drugs effectively (NHS, 2013).
Physical examination often reveals an enlarged and smooth, but seldom tender, liver (NHS, 2013). Signs of chronic liver disease — such as spider angiomas, ascites, or asterixis — may be absent. Symptoms can be non-specific and mild, including loss of appetite, weight loss, painful or distended stomach, and nausea or vomiting. Physical manifestations may include hepatomegaly, jaundice, ascites, spider angiomas, fever, and encephalopathy. Alcoholic cirrhosis may present as decompensation even when fatty liver or alcoholic hepatitis did not precede it, and it may also be diagnosed concurrently with acute alcoholic hepatitis. The symptoms and signs of alcoholic cirrhosis are not always distinguishable from other causes of chronic liver disease. The patient may present with jaundice, pruritus, abnormal laboratory findings, or complications of portal hypertension such as variceal bleeding, ascites, or hepatic encephalopathy. Symptoms are often absent until the advanced stage (NHS, 2013).
