The Importance of Black Doulas in Promoting Improved Birthing Outcomes for Black Mothers

1. What is one important role of a Black Doula in effecting a positive outcome for Black Maternal Mortality and Morbidity rates?

It is reasonable to suggest that the birth of a child, especially for first-time mothers, is an emotional experience that is inherently fraught with anxiety and even terror over the potential outcomes due in large part to the unknowns that are involved. This is especially true of minority mothers who lack the same access to maternal care as their white counterparts (Sobczak, 2023). The unknowns that are involved in giving birth combine to create a sense of dread and anxiety that adversely affect the mental health and potentially physical health of pregnant black women. In this regard, Greco (2024) points out that, “Black women are fearful of birthing hospitals and the medical system during their pregnancies, and in the time leading up to their birth experiences” (para. 9).

This disparity in health care access and information can also translate into suboptimal clinical outcomes for black mothers. Indeed, a Greco (2024) notes that this disparity in health care access means that black women die from preventable pregnancy-related causes twice as often as their white counterparts. This disparity also underscores the importance of the role that is played by black doulas. For instance, Sobczk (203) according to, “Companionship and partner engagement during labor serve as emotional aspects of social support. By accompanying the mother, it gives them a sense of reassurance and confidence to manage anxiety and the stress that comes with labor” (p. 1).

Likewise, Greco (2024) also emphasizes the importance of black doulas in forging a therapeutic rapport that promotes improved birthing outcomes. In this regard, Greco (2024) advises that, “When you add the support and the advocacy piece of somebody who looks like you, somebody who represents you, somebody who is there on your behalf, it helps you to focus on your health, and focus on delivering healthy.” Moreover, there are also some important economic benefits to the use of black doulas that should be taken into account. As Greco (2024) concludes, “Black doulas are uniquely positioned to support Black birthing people, thereby impacting efforts to reduce racial disparities in birth outcomes …” (para. 9). Reducing racial disparities in birth outcomes will also translate into greater cost savings for American taxpayers through savings on preventable maternal-related mortality and morbidity outcomes for black mothers.

Achieving racial equity in reproductive care is an essential step for black women and the recognition of the invaluable role played by black doulas therefore represents important progress in this direction. By providing culturally competent support, advocacy, and companionship to black mothers who face systemic barriers to adequate maternal care, distrust in the medical system, and disproportionately higher maternal mortality and morbidity risks, black doulas can promote a therapeutic rapport, mitigate biases, and promote improved mental and physical health outcomes. In sum, the presence of black doulas for black mothers empowers and uplifts the community, addresses longstanding critical disparities in maternal and child health, and represents a meaningful stride towards broader racial equity and social justice in health care in the United States today.
