(4) Were hands scrubbed by using a brush and were fingers washed between?

(5) Were fingernails cleaned under?

(6) Were palms and wrists scrubbed/

(7) What was the total time for scrubbing hands?

(8) Were hands rinsed in lukewarm water for 10 seconds?

(9) Were hands dried with paper towels from hands to wrists?

(10) Was paper towel used to turn off water?

The questionnaire will measure the effectiveness of the instruction provided on hand washing through assessing the participants understanding of the proper hand washing procedures.

While formal assessments are standardized and result in scores being assigned or compared and may be in the form of summative type testing and possibly extend beyond the classroom environment, informal assessments are non-standardized and do not result in scoring or comparisons and may involve observation and interviews taking place in the normal classroom environment. Informal testing is reported most useful when attempting to gain knowledge in regards to the cognitive, manipulative, and affective abilities of the student. "The results of formal examinations when not enough to provide a concluding mark for the performance of a student often results in the conduction of informal assessments.
