Cardiovascular Surgery: Diagnoses, Medications, and Nursing Care

~9 min read 5 sections Health · Nursing Care
Abstract

This paper provides a comprehensive nursing overview of cardiovascular surgery, examining indications and risk factors for bypass graft, aortic replacement, and valve replacement procedures. It identifies and defines eleven common diagnoses encountered in cardiovascular surgical patients — including coronary artery disease, STEMI, NSTEMI, aortic aneurysm, aortic stenosis, mitral valve disease, and several comorbidities — and maps each to its relevant nursing concepts. The paper then reviews seventeen perioperative medications, detailing their indications, dosages, mechanisms of action, and side effects. Finally, it outlines essential post-operative nursing assessments and patient self-care education strategies for the first 24 hours and beyond.

Key Takeaways
  • Introduction and Risk Factors for Cardiovascular Surgery: Risk factors and indications for cardiac surgical procedures
  • Common Diagnoses in Cardiovascular Surgical Patients: Eleven diagnoses common in cardiovascular surgical patients
  • Nursing Concept Mapping for Cardiovascular Diagnoses: Nursing concepts mapped to each cardiovascular diagnosis
  • Perioperative Medications: Indications, Actions, and Side Effects: Seventeen perioperative cardiac medications reviewed in detail
  • Post-Operative Nursing Assessment and Patient Education: Post-operative assessment priorities and patient self-care teaching

What makes this paper effective

  • The paper systematically organizes a large volume of clinical information — diagnoses, nursing concepts, and medications — using clear parallel structure, making it easy to follow and reference.
  • Each medication entry consistently covers indication, dosage, mechanism of action, and side effects, demonstrating disciplined use of a clinical framework across diverse pharmacological agents.
  • The concept-mapping section bridges clinical diagnoses with abstract nursing concepts (e.g., perfusion, cellular regulation, immunity), showing the student's ability to translate medical knowledge into nursing practice language.

Key academic technique demonstrated

The paper demonstrates systematic literature synthesis across a wide range of peer-reviewed sources, clinical databases, and authoritative health organizations. Rather than summarizing one or two sources, the student integrates evidence from multiple studies for each medication and diagnosis, creating a clinically grounded reference document appropriate for nursing education.

Structure breakdown

The paper opens with surgical risk factors drawn from the literature, then defines eleven cardiovascular diagnoses with cited sources, maps each to nursing concepts, surveys seventeen perioperative medications in consistent detail, and closes with post-operative assessment priorities and patient self-care education. This linear clinical progression — from pre-operative context through pharmacology to post-operative care — mirrors the actual sequence of nursing practice in a surgical setting.

Introduction and Risk Factors for Cardiovascular Surgery

The indications and risk factors for bypass graft, aortic replacement, and valve replacement surgery include age, previous heart surgery, serum creatinine level, peripheral or cerebral vascular disease, left main coronary artery stenosis, and left ventricular ejection fraction, among others (Gardner et al., 2004). Understanding these risk factors is essential for nurses who care for patients in the pre-operative, intra-operative, and post-operative phases of cardiovascular surgery.

Common Diagnoses in Cardiovascular Surgical Patients

The most common diagnoses observed in patients who have undergone cardiovascular surgery are described below.

Coronary artery disease is triggered by plaque formed along the walls of the arteries responsible for pumping blood to the heart and other body parts (Centers for Disease Control and Prevention, 2021).

Non-ST elevation myocardial infarction (NSTEMI) is a type of heart attack that is less severe than a full myocardial infarction, since the damage caused to the heart muscle is comparatively lower (Baptist Health, n.d.). It is most commonly caused by tobacco use, hypertension, low physical activity, the presence of diabetes, family history or genetic predisposition, being overweight, and certain underlying medical conditions.

ST-elevation myocardial infarction (STEMI) is caused by transmural myocardial ischemia that leads to myocardial injury or necrosis (Akbar et al., 2021). EKG changes, chest pain, and myocardial ischemia are confirmed diagnostic criteria, along with abnormal cardiac biomarkers.

Aortic aneurysm refers to a balloon-like bulge in the aorta, the large artery that carries blood from the heart through the chest (Centers for Disease Control and Prevention, 2021). When blood is forced through this bulge, it can cause the artery wall to split, resulting in blood leaks (dissection) or ruptures — among the most common causes of death associated with this condition.

Aortic stenosis (AS) occurs when the heart's aortic valve narrows and does not allow blood to flow fully into the main artery of the body, reducing overall blood flow (Mayo Clinic, 2021).

Mitral valve disease occurs when the mitral valve — lying between the left atrium and left ventricle — does not function properly, such as failing to close accurately, which may result in backward flow or leakage of blood into the left atrium (Mayo Clinic, 2020).

Type 2 diabetes is the malfunctioning of blood sugar regulation; when the body cannot effectively use glucose as fuel, excessive sugar circulates in the blood, creating a chronic condition (Mayo Clinic, 2021).

Chronic kidney disease involves the gradual loss of kidney function over time (Mayo Clinic, 2021). The kidneys become progressively less able to eliminate waste products, impairing overall body functioning.

Hyperlipidemia is a condition in which the blood contains too many fats or lipids (Cleveland Clinic, 2021). It can lead to blockages in blood vessels and elevated cholesterol, increasing the risk of heart attacks.

Obesity involves the excessive accumulation of body fat, creating a high risk for numerous dangerous health conditions (World Health Organization, n.d.). A body mass index (BMI) above 25 is generally considered to signal elevated risk.

Hypertension is the persistent elevation of blood pressure within the vessels (World Health Organization, n.d.). The increased force with which blood must be pumped against vessel walls weakens the heart gradually over time.

Nursing Concept Mapping for Cardiovascular Diagnoses

The nursing concepts associated with each of the diagnoses above are as follows:

The concept map hierarchy, from foundational comorbidities to primary surgical diagnoses, progresses as follows: Obesity → Hyperlipidemia → Type 2 diabetes → Hypertension → Chronic kidney disease → Mitral valve disease → Aortic stenosis → Aortic aneurysm → STEMI → NSTEMI → Coronary artery disease.
