Preventing Catheter-Associated Urinary Tract Infection in Hospitals

~13 min read 6 sections Health · Infection Control
Abstract

This paper examines catheter-associated urinary tract infection (CAUTI) as a leading source of hospital-acquired infection, affecting a substantial proportion of catheterized patients. Using the case of C.W., a 78-year-old male patient who developed CAUTI during hospitalization, the paper applies the QSEN competencies of safety, evidence-based practice, teamwork, and patient-centered care to analyze how such infections occur and how they can be prevented. A literature review synthesizes research on catheter overuse, inadequate documentation, insufficient staff training, and limited patient education. The paper concludes with practice, education, and research recommendations aimed at reducing CAUTI rates through systematic policy improvements, nurse training, and informed clinical decision-making.

Key Takeaways
  • Introduction: CAUTI prevalence and QSEN framework overview
  • Case Study: Patient C.W. develops CAUTI from poor catheter care
  • Literature Review: Four studies on catheter overuse and infection prevention
  • Synthesis: Practice Recommendations: Systematic nursing steps to reduce CAUTI
  • Synthesis: Education Recommendations: Staff and patient education strategies for CAUTI
  • Synthesis: Research Recommendations: Gaps in CAUTI research and future directions

What makes this paper effective

  • Anchors abstract concepts in a concrete patient case (C.W.), making the discussion of QSEN competencies immediately applicable to a real clinical scenario.
  • Organizes the synthesis section into three distinct domains—practice, education, and research—providing a clear, actionable framework for CAUTI prevention.
  • Draws on multiple peer-reviewed studies with varied methodologies (surveys, systematic reviews, randomized clinical trials) to build a well-rounded evidence base.

Key academic technique demonstrated

The paper effectively uses a case study as a narrative thread running through the argument. Rather than presenting the literature in isolation, it repeatedly references C.W.'s situation to show how each finding directly applies, bridging theory and clinical practice. This technique strengthens both the analytical and persuasive dimensions of the writing.

Structure breakdown

The paper opens with an introduction that contextualizes CAUTI within hospital infection data and introduces the QSEN framework. A case study section presents C.W.'s situation and analyzes the safety failures. The literature review covers four studies examining catheter overuse, monitoring gaps, and patient self-management. The synthesis section is divided into practice, education, and research subsections, each offering specific recommendations grounded in the reviewed evidence. A references list in APA format closes the paper.

Introduction

The Centers for Disease Control and Prevention (CDC, 2017) states that urinary tract infections stemming from catheter use are one of the most frequent causes of hospital-related infection, with three-quarters of all urinary tract infections acquired by patients in a hospital setting coming from catheter use. Since up to roughly one-quarter of all hospitalized patients are catheterized according to the CDC (2017), a significant proportion of individuals are placed at risk for infection. Understanding why catheterization leads so frequently to infection is the first step in addressing this problem.

A catheter is a small tube that is inserted into the urethra to provide a draining solution for the bladder, allowing patients to empty their bladder without leaving their hospital bed. In spite of how commonly catheters are used and how closely they are associated with infection, Fink, Gilmartin, Richard et al. (2012) note that catheter-associated urinary tract infection has never been a primary focus of hospital-acquired infection control programs.

This paper discusses the case of C.W., a 78-year-old male patient who was admitted with shortness of breath and later readmitted with catheter-associated urinary tract infection (CAUTI). The case is used to examine how the QSEN competencies of patient-centered care, evidence-based practice, safety, and teamwork can each play a role in preventing and reducing the rate of CAUTI in hospitals. Safety, teamwork, patient-centered care, and evidence-based practice all combine to create an effective, efficient, and continuous care environment that protects patients from unwanted infection. The priority QSEN competency, however, is safety—as it is of paramount importance for both patients and providers.

Synthesis: Practice Recommendations330 words
Prevention of CAUTI can be achieved by taking systematic steps to raise awareness among nurses of the risks associated with applying catheters when not needed, of leaving catheters in for too long, of not monitoring catheter usage on a day-to-day basis, and of not documenting catheter usage so that physicians are made aware of its application. Practice should be systematically improved through the development and implementation of…

Unfortunately, studies show that many health care facilities lack effective guidance and policies on preventing CAUTI, which poses a problem for nurses who derive much of their practice-based education from the standards expected of them at their facility. The studies examined in the literature review indicate that more research is needed on how to reduce incidences of CAUTI. Clinical trials have established a relationship between unnecessary catheter use and…
