A rapid critical appraisal refers to determining a study's worthiness based on its applicability, reliability, and validity to clinical practice. In general, a rapid critical appraisal relies on some form of checklist to assist the reviewer in evaluating the effectiveness of a study and whether it can be applied to patients. Although multiple rapid critical appraisal tools are available, most respond to three very critical questions to determine a study's worth (Grove & Gray, 2018). These questions are: What is the validity of the results? What were the results, and are they meaningful? Do the results respond to the PICOT question and would they assist patient care within the clinical setting?
Evaluating Validity and Reliability in Research
Validity is the accuracy of the results. It seeks to establish how accurate study results are and whether they can be trusted (Ferrell et al., 2018). To determine this, the selection of participants should first be evaluated. For instance, what method of selection was used—were participants chosen randomly or through other means? Were mechanisms in place to minimize biased outcomes based on participant selection criteria?
Additionally, there is a need to assess the completeness and accuracy of the data—for example, whether the study's authors used a reliable and valid measurement instrument such as a survey. There is also a need to assess what kinds of data were included and what were excluded (Castellani et al., 2018). Furthermore, it is worth considering whether the researcher took practical steps to ensure complete and accurate data. Reliability, therefore, refers to the consistency and dependability of the results—that is, whether the results obtained accurately represent what is being studied.
Interpreting Results: Effect Size and Clinical Significance
Another critical component of rapid critical appraisal asks whether the results are meaningful. In other words, the question seeks to determine what the study established and how significant those findings are. To interpret results, terminologies such as effect size and significance level must be clearly understood in order to evaluate treatment effects (Ferrell et al., 2018). Effect size describes the strength of the relationship between variables; a higher effect size translates to a stronger relationship.
On the other hand, the level of significance refers to the probability of an event occurring or not occurring. The p-value typically represents this probability in research studies. When the p-value is smaller, there is a lower probability that the reported results occurred due to chance or a fluke (Castellani et al., 2018). As a result, the author can be more confident in the significance of the reviewed data (Grove & Gray, 2018). Most researchers adopt a p-value of 0.05 as the threshold for clinical significance.
Furthermore, it is essential to determine how likely and plausible the results are (Ferrell et al., 2018). For instance, are the study results consistent? Can they be reproduced? If the claims appear unrealistic, similar studies should be evaluated to determine whether the same results are obtained; otherwise, the intervention may not only be unhelpful but potentially dangerous to patients.
Another critical aspect of rapid critical appraisal is whether the results answer the PICOT question in a way that would enable effective patient care within the clinical setting. This is also referred to as clinical significance (Castellani et al., 2018). To evaluate whether a study is of clinical significance, one must consider several questions that depend primarily on clinical knowledge and understanding of the patient population. First, do the results apply to the patients under care? One must evaluate whether the patient population in the study resembles those in the clinical setting.
In most cases, differences are likely; however, one must understand what key differences exist and whether they could affect the intervention's effectiveness for the patient population. Second, what are the treatment's benefits and risks? In some research, authors can control and monitor risks carefully. However, in the clinical setting, risks may supersede benefits for the patient population because monitoring and control may not be as rigorous as in the original study (Castellani et al., 2018). A third question considers whether the treatment is practical within the clinical setting. Finally, one must determine the patients' values and expectations regarding the results.
