Exercise may benefit pulmonary patients in a variety of ways; however, pulmonary patients face many unique challenges to exercising that patients with other diseases do not. Pulmonary disease can affect the lungs in a variety of ways and includes respiratory disorders such as emphysema, chronic bronchitis, and pulmonary hypertension.

Two studies provide particular insight into the implications of exercise for this population. In "Comparison of Specific Expiratory, Inspiratory, and Combined Muscle Training Programs in COPD," published in Chest Journal, Dr. Weiner and colleagues explore the idea that weakness within the respiratory muscles may contribute to shortness of breath, which in turn may limit the ability of patients with this disorder to exercise regularly at a significant intensity (Weiner, 2003). In a second article, "Comparison of Effects of Strength and Endurance Training in Patients with COPD," published in the American Journal of Respiratory and Critical Care Medicine, Francisco Ortega and colleagues examined the effects of different exercise modalities as therapeutic agents for patients with chronic pulmonary disease. Each article presents a unique consideration for the implications of exercise in the physical therapy treatment of pulmonary disease patients.

Weiner et al.: Respiratory Muscle Training in COPD

The results indicated a significant increase in muscle strength and endurance for both inhalation and exhalation among patients in the non-control groups, with the inspiratory training group showing the greatest gains (Weiner, 2003).

Ortega et al.: Strength vs. Endurance Training

In the Ortega study, participants were enrolled in a 12-week exercise program in which they engaged in either strength training, endurance training, or a combination of the two. The study revealed that while both strength training and endurance training produced improvements, increases in submaximal exercise capacity were significantly higher for the endurance group than for the strength training group (Ortega, 2002). Importantly, any type of training modality was shown to improve breathlessness scores and perceptions of shortness of breath among participants (Ortega, 2002).

The most effective exercise method for individuals with COPD in this study was found to be a combination of strength and endurance training (Ortega, 2002).
