Background of the Problem
The Flint water crisis is an example of an emergency or disaster with considerable public health concerns that carry significant implications for nursing. The significance of this problem lies in its ability to highlight important lessons that can enhance the practice of nursing, nursing management, and nursing administration going forward. As noted above, the crisis originated in April 2014 following the decision by Flint officials to change the region's water source. The situation has been described as a scenario of unpreparedness, environmental injustice, government failure, inaction, intransigence, and delay (Flint Water Advisory Task Force, 2016).
The Flint water crisis was a by-product of ineffective disaster management approaches that generated serious nursing and public health problems. While city officials decided to change the water source following a series of events, they did not adopt suitable measures to ensure the new source was safe. For instance, the Michigan Department of Environmental Quality (MDEQ) incorrectly determined that corrosion control — a crucial component of water treatment — was not immediately necessary. Consequently, once water distribution began, city residents started complaining about its taste, odor, and appearance. This was followed by several water quality problems and operational difficulties that led to violations in water quality standards, including E. coli contamination and disinfection by-products (Flint Water Advisory Task Force, 2016). In addition, the corrosiveness of the untreated water increased the risk of Legionella contamination through lead leached from water pipes and plumbing fixtures.
The lead contamination and other challenges in Flint's water supply created serious public health concerns and dangers. Because the Flint River was the primary water source for the city's residents, a significant portion of the population was exposed to serious health risks. For instance, the water crisis exposed approximately 12,000 children in Flint to elevated levels of lead, which is associated with numerous long-term health problems. The severity of the crisis was further demonstrated by an outbreak of Legionnaires' disease that affected nearly 100 residents and killed 10 of them. Together with MDEQ, MDHHS persistently dismissed and discredited reports of lead contamination and the Legionnaires' disease outbreak. The disaster was prolonged by these actions, as well as by the rush by the city's water department to fully utilize the Flint Water Treatment Plant despite its unpreparedness. Furthermore, the U.S. Environmental Protection Agency (EPA) delayed enforcement of the Lead and Copper Rule and the Safe Drinking Water Act. Compounding the crisis was the failure of the Governor's office to take measures to reverse the erroneous decisions made by state-appointed emergency managers and MDEQ. These collective failures contributed to severe public health impacts, including increased lead exposure among residents, the Legionnaires' disease outbreak, and a range of other adverse health effects.
