They also help open blood vessels wider, thereby reducing blood pressure and improving blood flow. This improves the ejection fraction (the percentage of blood pumped from the left ventricle per heartbeat) and slows the progression of heart failure. Diuretics lower blood pressure and reduce body swelling (edema), allowing smoother blood flow through the veins. ACE inhibitors help prevent further damage to the heart following a heart attack or episode of heart failure.

Common side effects associated with diuretics, ACE inhibitors, and beta-adrenergic blockers include dizziness or lightheadedness, which can also cause the patient to feel fatigued—particularly after the first dose. ACE inhibitors are especially associated with a persistent cough, sore throat, mouth sores, confusion, and swelling or skin rash. Diuretics are associated with frequent urination that may last up to four hours after each dose, along with thirst and dehydration related to that increased urination, skin rash, fever, sore throat, cough, confusion, nausea, vomiting, and muscle cramps. Patients taking ACE inhibitors should avoid salt substitutes, which contain potassium and can cause the body to retain excess potassium. Over-the-counter non-steroidal anti-inflammatory drugs (NSAIDs) such as aspirin, ibuprofen, and naproxen should also be avoided, as they can cause the body to retain too much sodium and water and may diminish the drug's effectiveness. There are no specific additional instructions for patients taking beta-adrenergic blockers. Patients taking diuretics should consider taking their last daily dose (if on a twice-daily schedule) no later than 4:00 p.m., so that normal body functions can be restored during the evening and overnight hours.

Beyond the registered nurse (RN), the healthcare discipline best suited to support medication adherence is the pharmacy team. The entire pharmacy team is responsible for medication management and plays a key role in educating patients and improving adherence (Sharma & Taylor, 2012). The pharmacy team collaborates with the RN by educating the patient about each medication—how it works and what side effects to expect—so that both the patient and their family understand the importance of adherence. The team can also provide supportive materials such as leaflets and handouts to reinforce that understanding.

Dietary Modifications for Heart Failure

As part of heart failure treatment, patients should make specific dietary modifications to better manage their condition. The first modification concerns fluid intake. Patients with severe congestive cardiac failure (CCF) should restrict their fluid intake considerably, consuming only the amount necessary to avoid dehydration—particularly important for patients taking high doses of diuretics—while also reducing the risk of fluid overload and related complications.

Patients should obtain adequate nutrition from a balanced diet. Obese patients should be supported in reducing caloric intake to promote weight loss, while cachectic patients should be assessed by a dietician to determine their optimal intake for their body weight. Patients taking ACE inhibitors should avoid foods with high salt content and should not add salt to their meals. Experts recommend that these patients consume no more than 2–3 grams of salt per day. Generally, heart failure patients should also avoid low-salt food substitutes, as these products are often high in potassium and may cause problems with potassium retention.

Patient adherence is a serious concern when informing patients about required dietary changes. While most patients recognize that dietary modifications are necessary to prevent recurrence of heart failure, some resist making those changes. One expected obstacle involves cognitive factors: some patients understand what they are supposed to do but lack the motivation to follow through (Chen, Tseng, & Cheng, 2013). Their willingness to adhere may be present while their capacity to act on that willingness is limited.

A second obstacle is language barrier. Language barriers frequently inhibit patient comprehension, and literacy and comprehension are central to a patient's willingness and ability to adhere. A third obstacle is the patient's health beliefs. When required dietary changes conflict with a patient's existing health beliefs, adherence becomes difficult because the patient does not believe in the necessity of those changes. Finally, the physician–patient relationship is central to adherence. When the physician actively encourages and educates the patient, the patient feels more motivated to comply. Cohesive partnerships between patient and physician help patients follow recommendations they perceive as mutually agreed upon.

Nutrition professionals can address the factors that affect a patient's ability and willingness to initiate and sustain dietary changes, because they are equipped to educate patients and correct common misconceptions that undermine adherence. They also have the deepest understanding of the patient's specific dietary requirements and can provide targeted suggestions for improving nutritional status. This team member collaborates with the RN by counseling the patient and organizing dedicated sessions to address concerns, ensuring the patient understands the rationale behind each dietary change and develops genuine commitment to following it.
