Hepatitis B virus (HBV) is a major public health problem across the globe, contributing to nearly one million deaths annually. It is an infection that primarily attacks the liver and has the potential to cause both acute and chronic disease. This communicable disease is most commonly transmitted from mother to child during birth. Additionally, HBV is transmitted through contact with body fluids such as blood. Because it is a life-threatening condition, this viral infection has received considerable attention in the healthcare sector. Healthcare professionals and other relevant stakeholders have developed various initiatives to help reduce the prevalence of this infection and mitigate its impacts worldwide. This paper analyzes the communicable disease, the determinants of health relating to its development, the epidemiological triad, and the role of the nurse practitioner in the management of the infection.
Analysis of the Communicable Disease
The World Health Organization (2019) defines hepatitis B virus as a viral infection that attacks the liver and has the potential to cause acute and chronic diseases. The two common modes of transmission are perinatal transmission (from mother to child during birth) and horizontal transmission (through contact with body fluids such as blood). The infection is also spread through piercing, needlestick injury, exposure to infected body fluids and blood, and tattooing. Because HBV is a potentially life-threatening condition with high prevalence rates, it represents a major public health problem worldwide. HBV can cause chronic infection and increases the risk of death from liver cancer and cirrhosis. The infection results in considerable human morbidity and mortality due to the effects of chronic disease (MacLachlan & Cowie, 2015).
Most newly infected cases of HBV are asymptomatic, while some individuals experience acute illness with symptoms lasting several weeks. These symptoms include dark urine, abdominal pain, vomiting, extreme fatigue, nausea, and jaundice (yellowing of the skin and eyes). Probable complications associated with the infection include acute liver failure, liver cancer, chronic liver infection, and cirrhosis. Because there is no specific treatment for the infection, existing care approaches focus on maintaining comfort and adequate nutritional balance. These approaches include medications using oral antiviral agents such as tenofovir and entecavir, as well as lifestyle changes such as avoiding alcohol.
MacLachlan and Cowie (2015) note that HBV is associated with geographically diverse epidemiology, reflecting variations in age, population prevalence, mode of acquisition, and the probability of disease progression. Perinatal transmission accounts for the majority of cases of this viral infection. High prevalence rates are evident in Asia-Pacific and sub-Saharan African countries. The prevalence rate in the United States is estimated at nearly 0.4% (Kim, 2012). People aged between 25 and 44 years have the highest incidence rates of HBV at 4.0 per 100,000, while children have the lowest at 0.1 per 100,000. The incidence rate is also higher in men (2.7 per 100,000) than in women (1.6 per 100,000). While the United States has witnessed a steady decline in HBV incidence and prevalence rates across all racial and ethnic groups, Asian/Pacific Islander populations have the highest rates, followed by non-Hispanic Blacks and Alaskan Natives. Asians have the highest prevalence rate of HBV infection at 2.74%, followed by non-Hispanic Blacks at 0.64% and non-Asian/non-Blacks at 0.15% (Kim & Kim, 2018).
Determinants of Health
According to Healthy People 2020, health status is influenced by various personal, social, economic, and environmental factors. These factors fall under five broad categories: individual behavior, biology and genetics, social factors, policymaking, and health services. The development of HBV infection is influenced by several determinants of health. Social determinants that affect HBV infection include barriers to testing, cultural factors, ecological factors, lack of access to medical care, and country of origin (Greene et al., 2017). Additionally, biology and genetics play a role in the development of this viral infection. According to Zhang et al. (2019), the genetic diversity of several genes associated with intrinsic and adaptive immunity is linked to HBV infection.

Hepatitis B is a viral infection that has become a major public health problem worldwide. The infection, which attacks the liver, is a communicable disease most commonly transmitted from mother to child during birth and through contact with body fluids such as blood.
