Medical technology is applied across many areas of healthcare, including medical evacuation. Medical evacuation is an important part of healthcare, required when a medical issue cannot be effectively or adequately addressed at the patient's current location. This paper examines the history of medical evacuation in relation to technology development and implementation in this area.
Overview of Medical Evacuation
Medical evacuation is a process that has traditionally been utilized in the healthcare sector to help address the clinical needs of patients. The term medical evacuation refers to the process of moving a patient from one location to another for a higher standard of care. This process is carried out when the patient's clinical condition cannot be adequately addressed or treated at the current location. In such cases, the patient is moved to a location where he or she can receive quality medical care. According to Reksoprodio et al., medical evacuation is commonly known as MEDEVAC and entails the timely and efficient transfer of casualties to another location for quality care (par. 2). While en route to the new location, patients receive medical care, especially if they are wounded or require urgent attention.
In today's healthcare setting, medical evacuation involves the use of aircraft and other modes of transport for the speedy movement of patients to locations where they can receive a higher standard of care. The increased use of aircraft represents one of the major developments in this field. Air medical evacuation is usually a complex and challenging process that can be further complicated when a patient is suffering from a hazardous infectious or communicable disease (Gibbs et al., p. 359). However, air and aerial medical evacuation has improved in recent years because of the ease of air travel, the expansion of global commerce, and increased tourism. Even though these improvements have exposed all regions across the globe to diseases, they have also made it easier to evacuate patients to new locations for a higher standard of care. Medical evacuation is common in situations where there is limited local infrastructure to provide care, where there are potential negative impacts on local healthcare workers, where there is active armed conflict, or where there are local or national political concerns.
Early History of Medical Evacuation
Medical evacuation has developed to become an important part of today's healthcare sector and medical practice. Throughout the history of the healthcare field, patients have always been moved from one location to another in pursuit of a higher standard of care. Medical evacuation is particularly common in situations of active war or conflict, natural disasters or emergencies, and disease outbreaks. Despite medical evacuation existing for as long as organized healthcare, aerial medical evacuation initially began in military and paramilitary operations before being incorporated into non-military medical care. The emergence of aerial medical care in military settings and its later integration into civilian emergency medical care is attributable to a series of historical developments in medical transport. These developments paved the way for aerial medical evacuation to become common in non-military and emergency medical care settings.
The history of today's aerial medical evacuation in emergency medical care can be traced back to aerial medical evacuation in military and paramilitary operations. According to Olson, the United States Army pioneered air medical evacuation of casualties from the battlefield following the advent of the helicopter (p. S130). However, medical evacuation itself began in the first century — long before the helicopter was invented.
The application of medical evacuation can be traced to the first century, when casualties from the battlefield were evacuated to other areas for urgent, high-quality medical attention. During this period, the Byzantine Empire's army integrated a system for battlefield casualty evacuation (known as CASEVAC) using medics called Scribones. These medical professionals were stationed approximately one hundred meters behind the battlefield and were paid a gold piece for every casualty they rescued. Organized CASEVAC did not become well-documented, however, until the 18th century. During the French Revolution, Baron Dominique-Jean Larrey, Napoleon's surgeon, rode into battle on a horse-drawn carriage to evacuate injured soldiers, transporting them to the rear of the combat zone where they could receive treatment.
Prior to this period, battlefield casualties were largely considered liabilities to the war effort and were left for dead with little to no medical attention. There was minimal interest in medical evacuation because of various factors, chief among them the limited state of medical treatment at the time. Medical care during this period offered little hope for treatment or cure, making it difficult for any war casualty to survive or recover from battlefield injuries. Additionally, wounded soldiers were largely viewed as hindrances to the progression of the war and future military operations (Lam, p. 19-1). Medical evacuation also received little attention because it would require removing other combatants from the battlefield to serve as transporters. Consequently, the ability to move patients from the battlefield for medical treatment developed more slowly during the 18th century than other medical capabilities. Evacuation was not regarded as a medical responsibility, since healthcare professionals of the era treated only those who could reach their facilities on their own or were brought by others.
A significant turning point in the history of medical evacuation occurred in 1862 during the Civil War. Dr. Jonathan Letterman established a medical evacuation system using military ambulances for the U.S. Army. The development of automobiles during this period played a critical role in shaping this system and changed the history of medical evacuation considerably. Letterman's ambulance system for CASEVAC was seemingly influenced by Larrey's earlier work. Larrey had observed that most wounded soldiers died from lack of timely assistance and therefore designed the "Flying Ambulance" — a light wagon used to carry surgeons directly to the casualties rather than the other way around. Larrey successfully developed and equipped three divisions of ambulances, each with evacuation vehicles, medical equipment, and trained personnel.
Letterman's ambulance system for the U.S. Army during the Civil War was a significant turning point in the practice of medical evacuation. Following Larrey's contributions, most armies across the globe had established some form of evacuation system by the mid-1800s. Letterman's work, however, paved the way for much better military ambulance systems, since it capitalized on new technologies that enabled safe patient movement without compromising the overall war initiative. Moreover, he leveraged growing social activism that advocated for medical care for wounded soldiers. From this period until the mid-20th century, various forms of ground ambulances were used to evacuate casualties from the battlefield to locations where they could receive medical treatment.
