History of Nurse Practitioners in Canada and Alberta

~4 min read 4 sections Health · Nurse Practitioner
Abstract

This paper traces the history of nurse practitioners (NPs) in Canada, beginning with the role's introduction in the 1960s and 1970s in response to physician shortages, the launch of universal public health insurance, and a growing emphasis on primary healthcare. It examines early controversies surrounding NP education and professional positioning, the failed integration of the role in the 1970s, and the eventual resurgence and standardization of NP programs toward Master's-level preparation. The paper also provides a focused history of the nurse practitioner profession in Alberta, including the province's legislative milestones, the temporary cancellation of university programs in the mid-1980s, and the subsequent expansion of NP scope of practice through the 1990s and beyond.

Key Takeaways

✍️ How to write this paper — guide, tools & examples ▾

What makes this paper effective

  • The paper maintains a clear chronological structure, making the development of the NP role easy to follow across distinct historical periods.
  • It connects broad national trends to a specific provincial case study (Alberta), giving the analysis both scope and regional depth.
  • The paper identifies concrete causal factors — such as physician shortages, funding gaps, and legislative changes — to explain both setbacks and progress in the NP profession.

Key academic technique demonstrated

The paper demonstrates effective use of historical causation analysis: rather than simply listing events, it explains why the nurse practitioner role succeeded or failed at each stage. For example, the failed 1970s integration is attributed to a specific cluster of causes including funding gaps, perceived physician oversupply, and lack of legislative support — showing how policy, economics, and professional politics interact in healthcare history.

Structure breakdown

The paper opens with a national overview of advanced practice nursing in Canada, then narrows to the NP role's contested early years, followed by a section on the evolution of educational preparation. It concludes with a focused provincial history of nurse practitioners in Alberta. This funnel structure — broad to specific — is a classic and effective approach for a historical survey essay at the undergraduate level.

Introduction to Advanced Practice Nursing in Canada

Advanced practice nursing is one of the fundamental components of the Canadian health sector and has continued to experience tremendous growth in recent years (Kaasalainen et al., 2010). Nurse practitioners are among the major professionals in Canadian advanced practice nursing. Nurses have traditionally practiced in expanded roles in remote and rural areas across Canada, particularly in an informal capacity. The role of the nurse practitioner was first introduced in the country in the 1960s and 1970s. This introduction was attributable to various factors, including a perceived physician shortage, the launch of universal publicly funded medical insurance, increased medical specialization, and an increased focus on primary healthcare. The nurse practitioner role is one of the advanced practice nursing roles that emerged in Canada during this period due to the belief that nurses were suitably positioned to meet the nation's emerging healthcare needs.

Early Development and Initial Controversies

In the initial years of its introduction, the nurse practitioner role in Canada was characterized by widespread controversy regarding the education and positioning of these nurses within the healthcare system. This contributed to numerous published studies and initiatives established to help address the issue. For example, two groundbreaking randomized controlled trials conducted in Ontario in 1974 and 1975 demonstrated the effectiveness of the nurse practitioner role. However, integration of the nurse practitioner role in Canada failed in the 1970s despite strong evidence demonstrating its effectiveness. The failure was mainly attributable to the lack of funding for the nurse practitioner role, a perceived oversupply of doctors, inadequate public awareness of the NP role, lack of proper legislation, and insufficient support from health providers and policymakers (Kaasalainen et al., 2010). Despite this initial failure, most graduate nurse practitioners from Canadian university programs continued to practice between the 1970s and 1983, primarily in northern remote health facilities and community health centers.
