It is estimated that medical complications affect approximately 20 percent of patients with a hip fracture, and the majority of these are post-operative in nature.

Post-Operative Complication Risk Factors

The likelihood of a patient experiencing post-operative complications is generally influenced by the type of surgery, perioperative management, and the patient's pre-existing comorbid conditions. Anesthesia can cause many post-operative complications, most of which center on the airway, circulatory, or respiratory systems. There are also instances of patients reacting abnormally to anesthetic drugs.

Hemorrhage is another risk factor associated with post-operative complications (Carpintero et al., 2014). Hemorrhage can be classified as primary, reactionary, or secondary. Primary hemorrhage occurs when a vessel is cut during surgery; reactionary hemorrhage occurs when the patient's blood pressure rises at the end of the operation, causing previously stable vessels to begin bleeding; and secondary hemorrhage occurs mainly due to an infection that damages a vessel. Atelectasis — the collapse of areas of the lungs — may result in post-operative pulmonary complications such as hypoxia (Breathnach, Karip, McCoy, Cleary, & Quinlan, 2016). Post-operative pain must be assessed and managed promptly, as it is normally an indicator of an underlying problem. Before prescribing an analgesic, relevant patient-specific considerations must be taken into account.

Infection is another key risk factor for post-operative complications. Wounds should be checked consistently to ensure they are not infected, particularly within the first week after surgery. The wound should show no redness or discharge. Wound sinus, which occurs after apparently normal healing, is considered a late infectious complication (Carpintero et al., 2014). Surgical injury may also lead to complications through unavoidable tissue damage during the procedure, which can have far-reaching implications — for example, impotence following prostate surgery or urinary problems following abdominal procedures.

Potential Post-Operative Complications Following Hip Surgery

The major complication that may affect this patient after hip surgery is heart failure. This is primarily because he currently has high blood pressure; his recorded BP of 143/89 mmHg indicates Stage 1 hypertension. He is also at risk of deep vein thrombosis (DVT), which is the formation of blood clots within a deep vein, most commonly in the legs. DVT is a significant risk associated with surgery on the lower extremities such as hip replacement. If the patient remains immobile for a prolonged period, the calf muscles will not contract, impairing venous circulation and promoting clot formation. A decreased blood flow rate — referred to as venous stasis — can further predispose the patient to DVT (Whiting et al., 2015). Given the patient's hypertension, his blood is not circulating optimally, which increases the risk of clot formation throughout the body. To prevent this complication, it is essential to keep the patient active and to offer intermittent pneumatic compression where possible.

Post-operative pulmonary complications are defined as lung abnormalities that result in an identifiable disease adversely affecting the patient's clinical course. Pulmonary embolism (PE) is one potential complication associated with this system. PE is a blockage of a pulmonary artery caused by material — typically a blood clot — that has traveled from elsewhere in the body through the bloodstream. PE is most commonly caused by DVT and may manifest as breathlessness (sudden or gradual), chest pain that worsens with breathing, or sudden collapse (Carpintero et al., 2014). Because this patient is at risk of developing DVT, he is consequently at risk of PE. Prevention of DVT would also reduce the risk of PE. The patient's anticipated immobility following hip surgery further elevates this risk, as patients often find it difficult to mobilize and prefer to remain in bed rather than exercise.

Urinary tract infections (UTIs) are the most common nosocomial infection and affect approximately 12 to 61 percent of all patients who undergo hip surgery or sustain hip fractures (Lanting, Odum, Cope, Patterson, & Masonis, 2015). UTIs have been shown to prolong hospital stay by an average of 2.5 days and are associated with a higher mortality rate. The single most important risk factor for UTI is the urinary catheter. It is recommended that indwelling catheters be removed within 24 hours of insertion to reduce this risk. In this patient's case, the catheter remains in situ, and nursing staff should ensure its prompt removal.

Pressure injuries occur when there is an imbalance between the extrinsic mechanical forces acting on the skin and soft tissues and the intrinsic susceptibility of the tissue to breakdown (Frisch et al., 2014). The most frequent predisposing factor is acute hip fracture, and approximately 35 percent of pressure ulcers develop within the first week of hospitalization. A history of smoking further elevates this risk. As this patient is a heavy smoker, nursing care should focus on prevention through aggressive skin care and adequate nutrition to prevent ulcer formation.

Gastrointestinal post-operative stress ulcers are well-documented medical complications following hip surgery (Carpintero et al., 2014). Patients with a prior history of gastroduodenal ulcers are particularly susceptible following major surgery. To prevent gastrointestinal bleeding, proton pump inhibitors and antacids have both been shown to be effective and to minimize associated mortality and morbidity.

Because the patient is employed as a carpenter — a role involving prolonged standing and heavy lifting — he is at risk of musculoskeletal complications. These activities may aggravate the left hip if the tissues have not fully healed, increasing wear and tear on the joint. Additionally, smoking is known to impair musculoskeletal recovery and bone healing.

It is estimated that 10 percent of patients who undergo hip fracture surgery experience cognitive complications post-operatively (Carpintero et al., 2014), though most are mild. Delirium is one of the most common cognitive complications, and patients may present with either hyperactive or hypoactive cognitive status. Nurses must monitor for both presentations. In this case, the patient's Glasgow Coma Scale (GCS) score of 14 indicates that he is conscious and responsive to most interactions, which facilitates early detection of delirium should it develop.
