Overview of Clinical Experience: Clinical Log #2

Reflecting on the additional 50 clinical hours since my last log, I have witnessed significant personal and professional growth in my role as a psychiatric nurse practitioner (NP) intern. This period has been instrumental in deepening my understanding of psychiatric care, enhancing my diagnostic skills, and solidifying my therapeutic relationships with patients. Below, I will share insights into my continued development, my evolving relationship with my preceptor, and an update on a patient I have been closely monitoring.

Relationship with Preceptor and Progress Towards Clinical Goals

My relationship with my preceptor, AG, continues to thrive, offering a bedrock of support and education that significantly contributes to my learning. AG's mentorship has been pivotal in allowing me more autonomy in patient evaluation and management. During this period, I have been granted the responsibility of evaluating patients independently, which has markedly improved my confidence and clinical judgment. This step forward in my clinical duties reflects the trust AG has in my capabilities and the progress I have made under his guidance.

The patient I have been monitoring, a 65-year-old male with major depression and anxiety, has not shown the expected improvement. However, AG reassured me that this stagnation is part of the disease's nature, emphasizing the importance of patience and persistence in psychiatric care. This experience has taught me the value of managing expectations, both mine and the patients', and the necessity of adapting treatment plans based on patient response.

Clinical Site Evaluation

Mind Over Matter Health continues to be an exceptional setting for learning and practice. The blend of in-person and telehealth services has made psychiatric care more accessible and has also given me insights into the future of mental health services. My interactions with patients have become more nuanced and supportive.

Facilitators and Barriers to the NP Role

The supportive environment at Mind Over Matter Health has greatly helped my learning, with many opportunities to engage with patients and hone my skills. However, the challenge of adjusting to the unpredictable nature of psychiatric illnesses remains. My experience with the patient I monitor has shown the need for flexibility and adaptability in patient care strategies.

Self-Evaluation and Growth

Upon self-reflection, I recognize a significant enhancement in my ability to connect with patients and navigate their care journeys. My documentation skills, initially identified as an area needing improvement, have seen progress thanks to targeted practice and feedback from AG. This iterative learning process has been invaluable in my development as a psychiatric NP.

Looking ahead, I plan to continue refining my clinical skills and knowledge. I want to set weekly objectives that align with my goal of delivering empathetic and effective psychiatric care. My focus will be on further improving my documentation skills.

Patient Summary and Progress

The patient, LR, is a 65-year-old Caucasian male, who presents a challenging case of major depression and anxiety exacerbated by profound loneliness. Despite ongoing psychotherapy and a medication regimen including Lexapro and Hydroxyzine, LR's progress has been slower than anticipated. His social isolation, coupled with unsuccessful attempts at forming meaningful relationships, continues to be a significant barrier to his recovery.
