LETTER OF INTENT

I write this letter of intent to the DDA with the objective of enrolling as a HCBS Medicaid Waiver Provider operating within the ............................ area. The company is committed towards ensuring the best outcomes among persons diagnosed with various developmental and intellectual disabilities. Amongst other things, the company seeks to facilitate the successful integration of such persons back into the community. More specifically, the company is committed towards ensuring that persons diagnosed with developmental and intellectual disabilities are provided with the relevant support and equipped with the capabilities to take charge of their own lives and become productive members of the communities or societies in which they live. Towards this end, the range of services offered on this front are not only personalized, but also optimized to address the unique needs of those diagnosed with various developmental and intellectual disabilities.

My name is XXXX XXXX. Ive been an RN for a little over 15 years. During this period, Ive acquired a wide range of skills and capabilities on multiple fronts including, but not limited to; patient care, critical thinking, and team-work. A significant portion of my career has been spent in the ER where I worked as part of a wider team in the provision of emergency medical care and assessment of diverse patient needs. In the most recent 5 years, I have been working in mental health facilities specifically as a psychiatric mental health practitioner. It would be prudent to note that mental health happens to be part and parcel of the overall wellbeing. Towards this end, my role as a psychiatric...…be equipped with the skills and capabilities required to embrace the need for people to take charge of their own lives on the basis of their wishes and desires, and develop the rights systems and structures to advance this need.

In the final analysis, it would be prudent to note that the company is driven by an inherent aspiration to make a difference. This is more so the case with regard to the achievement of better outcomes for persons diagnosed with the various developmental/intellectual disabilities. We strongly believe that persons with developmental/intellectual disabilities should be facilitated to regain their membership in the community.
