Abstract

This three page paper is part of an overall PICO research plan designed to prevent the instance of CAUTI in hospitals. Catheter-related urinary tract infections are preventable but common. This intervention will change the ways nurses work, creating a team-based approach that ensures clear duties and roles. This will prevent CAUTI instances because the catheter remains in less time.

¶ … intervention for my PICO problem is changing the policies and procedures in hospital with the goal of reducing instances and severity of catheter-related urinary tract infections among affected patients. Although it is an administrative intervention, the outcomes are related to patient data.

The independent variables in the research include structure of nursing staff teams; the specific policies that are implemented including catheter insertion and removal procedures and regulations; and finally, the materials that are used in catheterization. Specific independent variables are listed in Appendix A.

Dependent variables include overall number of instances of CAUTI, tracked according to patient population and why the patient was catheterized. In addition to rate of CAUTI infections, the costs associated with the infection can also be a dependent variable. The material used to measure dependent variables is listed in Appendix B of this report.

Methods: Summary

The research begins with a careful design of an administrative memorandum calling a meeting of all nurses and nurse managers. This meeting will help establish new guidelines for employees working in areas that require knowledge of catheterization. The concept of team building will be central to the theme of the discussions during these nurse meetings.

After the initial meeting, nurses will work together designing a comprehensive employee rules and procedures handbook related specifically to catheterization. The rule book will be based on evidence-based practice guidelines, which are culled from the review of literature and research gathered for the PICO project. Issues related to specific procedures will be built alongside more important topics related to this PICO research, such as which staff member is responsible for monitoring the catheter; and ensuring the patient does not have a catheter in any longer than necessary. Because catheterization time is directly linked with CAUTI infection, the most important procedural goal is to reduce unnecessary instances of catheterization and reduce the overall time any patient spends with a catheter.
