The main effect that hypothyroidism has on the human body is a slower, more sluggish metabolism. However, to describe it only in those terms would be an oversimplification. The thyroid is responsible for controlling how the body uses energy from food — which is precisely what metabolism is. Metabolism affects body temperature, heart rate, how efficiently calories are burned, and much more. If a person's thyroid is not as active as it should be, all of these processes slow down and become more inefficient. This means less energy is available to the body and fewer calories are burned at rest, making it easier to gain weight and harder to lose it. The implications for obesity and its associated conditions — such as diabetes and heart disease — are considerable (WebMD, 2016).
Causes and Risk Factors
The most common cause of hypothyroidism is Hashimoto's thyroiditis, a condition in which the thyroid becomes inflamed as the result of an autoimmune attack by antibodies. A viral infection may or may not be present during this process. There are, however, other causes. These include radiation therapy (for example, as part of cancer treatment), radioactive iodine treatment, the use of certain medications, thyroid surgery, and insufficient iodine in the diet. Medications that may contribute to the condition include Cordarone (amiodarone), lithium, interferon alpha, and interleukin-2. Additional causes or aggravating factors include pregnancy (the precise reason is not fully understood), thyroid problems present at birth, pituitary dysfunction, and problems with the hypothalamus — a region of the brain.
Hypothyroidism manifests in one of two ways: primary hypothyroidism and secondary hypothyroidism. Primary hypothyroidism occurs when the thyroid gland itself is dysfunctional. Secondary hypothyroidism occurs when a problem outside the thyroid — such as pituitary dysfunction — disrupts the gland's hormone secretion (WebMD, 2016).
People at elevated risk of hypothyroidism include those of white or Asian descent, older individuals, people who experience premature graying of hair, and those with any autoimmune disorder such as type 1 diabetes or celiac disease. People with bipolar disorder are also at risk, as lithium — a common treatment for that condition — can interfere with thyroid function. Additional at-risk groups include those with Down syndrome and Turner syndrome.
Symptoms and Diagnosis
Symptoms of hypothyroidism are varied and can affect multiple body systems. They include changes in the menstrual cycle (in women), constipation, dry hair, hair loss, dry skin, fatigue, sensitivity to cold, a slower heart rate, swelling of the thyroid gland, unexplained weight gain, and carpal tunnel syndrome. Additional symptoms — particularly notable in infants — include cold hands or feet, extreme sleepiness, hoarse crying, poor growth, very low muscle tone, persistent jaundice (yellowing of the skin or eyes), poor feeding, a puffy face, abdominal bloating, and a swollen tongue.
Hypothyroidism is confirmed through blood tests. The two most commonly used are the thyroid-stimulating hormone (TSH) test and the T4 (thyroxine) test. A low T4 level indicates the presence of hypothyroidism. The TSH test is also useful because moderately elevated TSH levels suggest a milder rather than more severe case of the disorder (WebMD, 2016).
Pharmacological Treatment
The primary treatment for hypothyroidism is relatively straightforward: the patient takes synthetic (man-made) T4 hormone to supplement the body's insufficient thyroid output. The approach is analogous to a type 1 or type 2 diabetic taking insulin because the body does not produce enough of it on its own. Leaving hypothyroidism untreated is inadvisable, as it can lead to heart problems, infertility, joint pain, and obesity, among other complications. Women with hypothyroidism who are pregnant face additional concerns, since the developing fetus depends on the mother's thyroid hormone supply. If this is not addressed in utero, the child may experience mental developmental issues after birth. In extremely severe cases, very low thyroid levels can lead to a condition called myxedema — the most serious form of hypothyroidism — which can cause loss of consciousness, coma, or a potentially fatal drop in body temperature (WebMD, 2016).
Several synthetic T4 medications are available, including Synthroid, levothyroxine, Armour Thyroid, Levoxyl, Cytomel, Tirosint, and Thyrolar. The vast majority of thyroid medications are taken orally. The Mayo Clinic strongly endorses levothyroxine and cautions that careful dose management is essential — taking too much T4 can itself cause problems, including increased appetite, insomnia, heart palpitations, and shakiness. Starting at a low dose and gradually increasing it is often recommended. The cost of levothyroxine is relatively modest; for example, thirty tablets of 50 mcg cost approximately fourteen dollars without insurance, while the 200 mcg strength is approximately twenty-two dollars without insurance.
Certain supplements and medications can interfere with levothyroxine absorption and should be managed carefully. These include iron supplements, cholestyramine, aluminum hydroxide (found in antacids), and calcium supplements. Alternative preparations derived from animal sources — such as desiccated porcine thyroid — exist, but these are not regulated by the Food and Drug Administration and should be used with caution (Mayo, 2016).
Conclusion
If there are a few takeaways from the above, they would be the following. First of all, the diagnosis of hypothyroidism is fairly straightforward and easy to confirm once it is suspected. At the very least, it can be easily ruled out through TSH and T4 blood tests, allowing clinicians to identify whatever else may be affecting the patient. Second, standard synthetic T4 medications are the obvious and first-line treatment approach. While natural or alternative methods may be appropriate or practical in certain situations, conventional treatments for hypothyroidism are typically very effective, and there is little reason to avoid them unless the medication itself causes or worsens problems.
Generally speaking, healthy living and careful monitoring represent best practice. Just as a diabetic patient should closely track blood sugar and A1C levels, a patient with hypothyroidism should regularly monitor their TSH and T4 levels to ensure the condition remains well controlled.
