Ebrahim Haji Health Centre: Replacing an Aging Machine

The World Federation of KSIMC urgently sought donations from members around the world for the purchase of a new digital x-ray machine for the Ebrahim Haji Health Centre in Dar es Salaam, Tanzania (COEJ, 2011). The X-Ray Department at the health center operates six days a week and twelve hours a day. Physicians regularly use x-rays for the diagnosis of disease and injury and as an essential part of treatment.

The health center was faced with the dilemma of a limited budget alongside the need for a more reliable x-ray machine. The existing Fujitsu x-ray machine was thirteen years old and had been malfunctioning frequently. Problems encountered with its use included unavailability of replacement parts, frequent breakdowns, rising maintenance costs, and patient inconvenience when the machine failed. The number of x-ray services conducted progressively increased over the four years during which the old machine operated — even during periods when some patients had to obtain the service elsewhere. These conditions clearly demonstrated the need for an up-to-date and more reliable machine to meet the center's rising demand.

Field experts and the center's radiologists recommended replacing the old unit with a new Philips PCR Eleva Digital X-ray. The Central Medical Board approved the recommendation. Advantages of the new machine include the company agent's technical service backup and staff training in Tanzania, the ability to view x-ray images directly on a physician's computer, and the ease of transmitting those images to specialists anywhere in the world. Proper patient care is the foremost justification for the purchase of this x-ray machine (COEJ, 2011).

Sonoma Valley Hospital: An Emergency Upgrade

The x-ray equipment at Sonoma Valley Hospital was more than twenty-five years old when it broke down, prompting the hospital to request an emergency upgrade from the Sonoma Valley Health Care District (Charrier-Botts, 2011). Repair of the existing machine was not an option: its vendor was no longer in business, and the only technician with knowledge of the equipment was unavailable. The hospital maintained two x-ray rooms to accommodate patient imaging needs, but the breakdown of one room forced all patients to be directed through a single room (Charrier-Botts, 2011).

If the situation persisted, hospital board members warned that the facility risked losing its x-ray business entirely. Patients needed to continue being served in two separate x-ray rooms as before. As a matter of procedure, the finance committee was required to assess the proposal before making a recommendation to the board. Nevertheless, the board maintained that operations could not continue effectively without an equipment update (Charrier-Botts, 2011).

UC Davis Health System: Digital Radiology Workflow and Cost Analysis

A study at the UC Davis Health System analyzed workflow, operational costs, physician satisfaction, and self-recorded interactions among on-call radiology residents following the adoption of a Picture Archiving and Communications System (PACS) (Srinivasan et al., 2003). Prior to PACS implementation, physicians spent one to three hours per day searching for films and reported significant dissatisfaction with radiology services. After implementation, physicians began viewing and interpreting images independently, raising their satisfaction levels and enabling real-time reporting. Time spent consulting with radiologists fell by 90%.

The quantitative outcomes were substantial. Search time was reduced from one to three hours down to between two and sixteen minutes, saving the equivalent of 21.5 physician years — valued at approximately $1,034,150 annually. PACS reduced film printing by 73.4% and cut filing labor costs by 50.3%, representing an annual saving of $1,001,453.
