Etiology and Epidemiology

The etiology of amblyopia involves early errors in vision development. It causes visual deprivation resulting from strabismus (squint) or refractive defects, most commonly in young children's eyes (Wong, 2014). The weaker eye exhibits reduced neuronal responsiveness, disrupting the transmission of visual inputs relative to the normally functioning eye. This flawed interaction between cortical neurons — which creates an inhibitory connection — persists throughout the condition and is most prominently established during the early years of life. During this critical period, the risk of developing the disability increases if high levels of misalignment are detected or if the onset of strabismus occurs very early. Additionally, complications such as cataracts and ptosis that disturb the visual axis can also result in amblyopia.

The epidemiology of amblyopia reveals that its impact is both worldwide and lifelong. A global meta-analysis found that approximately 99 million people had this visual impairment in 2019, with projections suggesting the number could rise to 175 million by 2030 and 222 million by 2040 (Meng et al., 2021). Between 1% and 6% of children are affected by this developmental disability, while the prevalence rate among adults ranges from 1.43% to 5.64%. Prevalence rates are also known to vary according to geographic region, age, ethnic group, diagnostic criteria, illness severity, and gender. Other researchers have reported a global prevalence rate of 3.4%, with a notably higher rate in Africa (Mostafaie et al., 2020).

Course and Diagnostic Procedures

The course of amblyopia can vary considerably from child to child depending on the onset of the problem. As a developmental disorder caused by a deficiency in visual acuity (VA) in one or both eyes, it rarely involves actual organic eye disease beyond strabismus, anisometropia, or some form of visual deprivation originating in infancy or early childhood (Chen & Cotter, 2016). In addition, the amblyopic eye may be affected by a range of other visual deficits, including abnormal contour interaction, reduced contrast sensitivity, difficulties with positional sensitivity, distorted eye movements, abnormal spatial processing, suppression, and poor accommodation (Chen & Cotter, 2016).

Modern evidence-based research has challenged long-held beliefs about treatment timelines. Some findings indicate that certain forms of amblyopia can respond to treatment beginning within two hours of dosage, with increases in dosage possible when treatment of the moderate amblyopic eye is required. Patching dosage of two to six hours per day has proven effective in some cases when continued for a sufficient number of weeks.

Diagnosis of amblyopia is made by measuring visual acuity using an eye chart with optotype-based recognition tasks (Zagui, 2019). For children who are too young to respond verbally, behavioral methods such as fixation preference testing can be used. Clinicians observe the vigor with which a child uses one eye to fix on an object compared to the other eye. Quantitative grading is used to maintain accuracy and properly record fixation preference, since several distinct types of amblyopia must be correctly identified. Visual acuity may also be assessed using Teller acuity cards. It is recommended that optotypes — including numbers, letters, and symbols — be tested as early as possible to maximize reliability (Zagui, 2019).

Furthermore, the American Academy of Pediatrics recommends that visual acuity be assessed at every pediatric visit so that early detection and diagnosis of amblyopia can facilitate early treatment, which results in a successful outcome approximately 60% of the time (Zagui, 2019).

Medical Interventions

Early treatment can help children achieve clear vision and support better academic performance, since amblyopia severely impacts learning. The most common forms of treatment involve providing a clear retinal image to both eyes and correcting ocular dominance (Park, 2019). The broader treatment process includes addressing any underlying organic illness, prescribing accurate optical correction, and applying dominant-eye occlusion or penalization therapy within the appropriate sensitive developmental period. These measures are intended to restore normal or near-normal visual function by preventing eccentric fixation and promoting more widespread synaptic input to the visual cortex (Park, 2019).

The first and most critical step is providing a clear retinal image to each eye of the affected child. Evidence-based guidelines for refractive correction were outlined in the 2018 Preferred Practice Pattern guidelines published by the American Academy of Ophthalmology (Park, 2019). Diagnosis should be confirmed before optical correction is initiated, including a thorough review of the patient's history, visual symptoms, and physical examination of both eyes. Research has shown that optical correction in children aged 3 to 10 years with bilateral refractive amblyopia proved effective in approximately 70% of cases (Park, 2019).

Another method of treating amblyopia is patching, which involves covering the normally functioning eye in order to stimulate the affected eye (Park, 2019). This approach has been a primary treatment method for more than 250 years and remains preferred by many eye specialists. However, debate exists regarding the optimal number of patching hours per day, and various randomized controlled studies have been conducted on this question. One Pediatric Eye Disease Investigator Group (PEDIG) trial found that six hours per day was sufficient to achieve the level of correction needed for the amblyopic eye (Park, 2019). For children aged 3 to 7 years, six hours of daily patching is generally recommended, while two hours per day is considered sufficient for children with moderate amblyopia.

The third treatment method, penalization, involves administering atropine to the normally functioning eye to induce cycloplegia — temporary paralysis of the eye's focusing muscle (Park, 2019). This approach is appropriate for children who do not show adequate improvement with spectacles alone. Although cycloplegia carries some risk of transiently impairing the non-amblyopic eye, balancing accommodation in both eyes is necessary to achieve normal or near-normal vision. Atropine is particularly recommended when the non-amblyopic eye is hyperopic. PEDIG trials have compared the effectiveness of patching versus atropine penalization, finding both effective for children with moderate amblyopia. Improvements were seen more rapidly in the initial weeks, with the rate of improvement declining after six months. A separate PEDIG trial found that children aged 3 to 12 years with severe amblyopia also showed improved vision with atropine treatment (Park, 2019).
