The condition is generally thought to be associated with a simple imbalance between a person's energy intake and expenditure; however, more recent research demonstrates that behavioral, genetic, and physiological factors also play a crucial role in its etiology. Moreover, exercise and diet are considered important components in the prevention and treatment of obesity and its associated disorders, including diabetes, heart disease, and hypertension.
This literature review begins with an overall assessment of the overweight and obesity epidemic, including an overview of its worldwide prevalence, societal impact, and etiology. This is followed by a discussion of the risk factors and complications of obesity, analyzed in relation to treatment. Overweight and obesity represent a global epidemic: nearly 1.2 billion people worldwide are overweight, while at least 300 million suffer from obesity. Despite being one of the top 10 most preventable health risks globally, obesity results in the death of at least 300,000 people in the United States annually. The severity and impact of obesity are attributed to its association with other disorders such as liver disease, diabetes, and hypertension. Although the condition is commonly thought to result from a simple imbalance between energy intake and expenditure, it is also attributed to behavioral, genetic, and physiological factors (Wilborn et al., 2005, p. 4). It is estimated that the prevalence of obesity in established market economies, particularly in Europe, averages 15–20% of the population, demonstrating that the condition is a major health concern worldwide.
Beyond its global prevalence, obesity carries numerous societal impacts, most notably increased healthcare costs. Overweight and obesity heighten the risk of various life-threatening diseases such as cardiovascular disease and hypertension. Obesity is considered a major risk factor for heart disease because of its adverse effects on cardiac structure.
Risk Factors and Complications
According to Wilborn et al. (2005), the imbalance between energy intake and expenditure is the primary risk factor for obesity (p. 6). Excessive intake of energy nutrients contributes to an increase in the size and number of adipocytes at several stages of the lifespan. Energy expenditure encompasses the basal metabolic rate, physical activity, and the thermic effect of food. While increased energy expenditure and physical activity help prevent and treat obesity, this recommendation is often not adhered to, making the imbalance between energy consumption and expenditure the foremost risk factor. Morenga, Williams, Brown, and Mann (2010) state that increased intake of dietary protein and fiber reduces adiposity and some components of metabolic syndrome in overweight women, whereas insufficient intake enhances the risk of obesity (p. 1327).
Recent studies have also demonstrated that physiological, genetic, and behavioral factors can become major risk factors for obesity. These factors affect body mass index in significant ways. Genetic and environmental influences on eating behavior may increase the risk of obesity, which is sometimes driven by increases in portion sizes (Wilborn et al., 2005, p. 7). The role of these factors is evident in the measures undertaken to control energy homeostasis and in the growing use of lifestyle intervention programs that incorporate healthy nutritional practices and exercise training (Arciero, 2008, p. 757).
The complications of obesity are closely tied to the other disorders it promotes. Generally, obesity is a major risk factor for several chronic conditions, including hyperlipidemia, cancer, heart disease, hyperinsulinemia, and diabetes (Layman et al., 2005, p. 1903). It also elevates the risk of cardiovascular complications such as elevated insulin and blood glucose concentrations, high blood pressure, and unfavorable blood lipid profiles. Additional complications include arrhythmias, stroke, peripheral vascular disease, gallbladder disease, sudden death, arthritis, sleep disorders, and infertility. Overweight and obese men and women often find it difficult to maintain energy reduction and dietary fiber intake at optimal levels. For pregnant women, obesity creates complications that may result in premature birth or pregnancy loss.
Conclusion
Overweight and obesity have developed into major public health issues attributed to various factors, most notably the imbalance between energy intake and expenditure. While various treatment programs have been developed to address these risk factors, lifestyle programs are considered the most effective because of the role physiological and behavioral factors play in contributing to obesity. Studies have demonstrated that exercise and diet are the main components of these programs, as they contribute to significant weight loss, improved body composition, and preserved lean mass.
Although different levels of protein intake produce different results when combined with varying exercise programs, a high protein diet combined with moderate-to-high intensity exercise training is regarded as the most effective treatment approach for overweight and obese adults seeking to improve body composition and reduce associated health risks.
