For a successful reduction in the proportion of obese adults, obesity should be addressed in the early stages of adulthood, before patterns of behavior become deeply entrenched.

Prevalence and Factors Associated with Overweight and Obesity

The rates of overweight and obesity represent a global concern, with similarly high rates seen across various population segments — including married women and those with higher levels of education. The World Health Organization (WHO) estimated that 2 billion people, including 600 million adults, were obese in 2014. Obesity is more prevalent in economically developed countries, although prevalence rates in developing countries are projected to rise substantially between 2005 and 2030.

Individuals with a Body Mass Index (BMI) of 30 kg/m² or higher are more prone to the comorbidities commonly associated with obesity. In rural areas of America, prevalence rates are 3.6–7% higher than in urban areas, partly due to limited proximity to medical facilities. Cultural factors also contribute to higher prevalence rates in rural areas across the globe. The transfer of Western and urban culture into developing countries has been associated with the adoption of unhealthy dietary habits and sedentary lifestyles, further increasing obesity prevalence in those regions.

Part of the obese population consists of individuals who have lost weight but struggle to maintain that loss. In rural areas, men are less likely to exercise than women and tend to view health promotion behaviors as a sign of weakness, reflecting prevailing masculine norms. Current health status and existing stress levels are also critical variables, since environmental and genetic factors play a major role in determining the actual effects of any intervention.

Health Risks Associated with Obesity

Obesity is associated with a range of serious comorbidities, including type 2 diabetes, certain cancers, cardiovascular diseases, and breathing disorders. It negatively affects quality of life due to challenges such as respiratory disease and reduced physical mobility. There is also a markedly high rate of premature mortality among people living with obesity, underscoring the urgency of effective public health responses.

Technological and Behavioral Interventions

Mobile health interventions using internet-enabled devices, such as tablets and smartphones, facilitate the efficient delivery of individually tailored coaching, time-flexible feedback, and environment-specific health behavior change programs. Training obese individuals to use these technologies creates the capacity for remote engagement with weight loss support. Low-intensity interventions can be delivered through internet platforms and integrated with behavioral self-regulation strategies and self-weighing to produce scalable, flexible programs. Such technology-based behavioral weight-loss interventions are highly adaptable and can be implemented in rural areas with adequate training. Exercise programs can also be disseminated in digital formats — such as DVDs or streaming video — to support exercise regimens and motivation among geographically isolated populations.

Behavioral interventions are centered on self-monitoring to track exercise levels or daily caloric intake. Intensive behavioral counseling is a key component of effective weight management programs. Behavior-based weight-loss interventions in adults with obesity can lead to clinically significant improvements in health outcomes. Pharmacotherapy conducted alongside behavioral interventions has a greater impact on weight loss and maintenance over one to two years than behavioral interventions alone. The involvement of primary care clinicians — even in limited interactions within group-based programs led by lifestyle coaches or dietitians — reinforces intervention messages through brief counseling and is a formative element of successful weight management.
