Public Health Assessment Tool

The Community Health Assessment and Group Evaluation (CHANGE) is a tool developed by the Centers for Disease Control and Prevention (CDC) to help public health practitioners assess the needs of their communities and develop action plans to address those needs. The assessment consists of four main components: 1) community health status; 2) community assets and resources; 3) community health problems and concerns; and 4) community capacity for change. Each of these components is further divided into sub-components, which are then used to generate a report that can be used to guide community health planning. The CDCs CHANGE assessment is designed to be conducted on a regular basis, as it provides a snapshot of the current state of community health and can be used to track progress over time (Centers for Disease Control and Preventions, 2010).

The CHANGE assessment tool is a community-level health assessment that helps groups identify areas of need and develop action plans to improve community health. The CHANGE process begins with a community health needs assessment, which gathers data on the health of the community using surveys, interviews, and focus groups. This data is then analyzed to identify key health issues and disparities.
