This paper provides a literature review on opioid use disorder and the use of opioids for medical benefits.

Research studies indicate that opioids are prone to misuse and addiction, even as they offer medical advantages when used in healthcare settings to treat patients (Hoffman, Terashima & McCarty, 2019). Global use of opioids increased between 2001 and 2013. The misuse of this substance has been widely documented in Africa, while a public health emergency was declared in North America and parts of Europe. Controversies have surrounded its long-term use for pain relief, including concerns about loss of analgesic effectiveness, the potential for addiction, and drug diversion. Opioids are considered highly effective for cancer patients and those undergoing major surgeries. In the United States, two-thirds of the population was reportedly involved in opioid overdose during 2017, with deaths resulting from the crisis.

Opioid use disorder is one of the substance use disorders that compels an addicted person to use opioids with an overpowering desire (Dydyk, Jain & Gupta, 2021). Tolerance increases over time, and withdrawal symptoms worsen when use is discontinued. It is a chronic health condition that includes addiction to the substance, representing a distinct form of disorder. The disorder involves periods of exacerbation and remission in the need for the substance, contributing to a susceptibility to relapse that may be persistent. The pattern is considered similar to other substance use disorders, as signs and symptoms can worsen when relapsing is consistent. If adherence to substance use continues, the chronic risk of overdose, suicide, or trauma may also worsen. Conversely, when the risk decreases, treatment therapy can be effective and abstinence from opioids becomes achievable.

The term "opioid" refers to chemical compounds that bind to opiate receptors (Rosenblum et al., 2008). The alkaloids derived from the opium poppy include morphine and codeine. Semi-synthetic opiates — the most commonly known forms of which are morphine and oxycodone — are derived from naturally occurring opiates. The term "narcotic" is often used alongside opioids due to their legal designation, as law enforcement agencies have restricted their use to clinical settings.

According to the American Psychiatric Association's DSM-5, opioid use disorder is characterized by an uncontrolled desire for the substance and continued use regardless of social and health consequences (Dydyk, Jain & Gupta, 2021). Opioids have both systematic and chronic inflammatory effects on the central nervous system. Frequent use disrupts important daily life activities and professional obligations by generating powerful cravings. A strong desire for opioids is difficult to resist among those who become addicted. The immune system is weakened with prolonged use, and studies have shown that opioid use disorder (OUD) most likely affects those who have had adverse childhood experiences (Bryant, Eaton & Li, 2021). This is most commonly referred to as early life stress (ELS), and the condition can worsen to a chronic level of use compared to the general population. A thorough understanding of the substance is best achieved by interpreting its inflammatory properties alongside OUD and its biomarkers for prevention, diagnosis, and treatment.

Common Street Names and Routes of Administration

Common street names for opioids include heroin, morphine, codeine, fentanyl, and the synthetic opioid oxycodone (Dydyk, Jain & Gupta, 2021). In the United States, opioids have been commercialized for oral, transdermal, and intravenous administration (Rosenblum et al., 2008). Oral and transdermal opioids are used in clinical settings — for example, hydrocodone and acetaminophen, or oxycodone and aspirin — for pain relief purposes. Single-entity formulations commonly include fentanyl, morphine, hydromorphone, methadone, and oxymorphone.

The subcutaneous route of administration is considered an effective method for achieving favorable outcomes — not for treating substance use disorder, but for certain medical procedures such as cardiac surgery, cesarean delivery, major gynecologic surgeries, and pediatric procedures (Ackerman et al., 2018). Intravenous administration is used for pain control, as it can be delivered easily by injection. Oral administration is the preferred method for pain management, and education of medical practitioners and nursing staff is essential for its proper use.

A study was conducted to determine the preferred route of administration (ROA) for opioid intake among three methods: swallowing, injecting, or snorting (Young, Havens & Leukefeld, 2010). Among urban participants, oral use was the most preferred method, in contrast to rural users who preferred snorting. Injection was most common for hydromorphone and morphine. This pattern was observed in nonmedical opioid use; however, previous studies indicated that in medical settings, methadone, morphine, and hydromorphone were also administered through alternative routes such as snorting, injecting, and chewing (Young, Havens & Leukefeld, 2010).

Previous literature corroborated that people who used opioids through non-oral routes were more likely to meet DSM-IVR criteria for substance use disorder or dependence (Surratt, Kurtz & Cicero, 2011). The treatment status of these patients was also reflected in their non-oral routes of administration, as the development of serious drug-related problems was more likely.
