Smoking remains the leading cause of preventable illness and death in America (CDC, 2014). It can lead to many cancers, heart disease, stroke, pregnancy complications, and — most critically — chronic obstructive pulmonary disease (COPD). COPD alone accounts for 443,000 deaths per year in America. Smoking cessation improves health by reducing the likelihood of developing these deadly illnesses.

Smoking also adversely affects workplace productivity, increases absenteeism and the use of disability or sick leave, and raises healthcare costs. The most recent statistics indicate that approximately $96.8 billion per year is lost through reduced productivity due to illness and early death. Male smokers become ill four times more frequently per year than non-smoking men, and female smokers experience two additional sick days per year compared to non-smoking women. Adult American male smokers spend $15,800 more and female smokers $17,500 more on treatments than their non-smoking counterparts (CDC, 2014).

The Role of Needs Assessment in the WHO Program

The Smoking Cessation Program recommended by the World Health Organization (WHO, 2012) is designed to assist organizational stakeholders in adopting their own cessation programs. Conducting a needs assessment before designing and developing a program will identify and prioritize goals, mechanisms, and actions for effective implementation. Stakeholders can thereby create precise strategies for addressing smoking issues. At the same time, these strategies will help manage inequalities among smokers and improve their overall well-being (WHO, 2012).

The involvement of stakeholders during needs assessment is paramount (WHO, 2012). Their input is invaluable as a source of innovation in the program. Stakeholders can help uncover and clarify complicated views and entrenched habits within organizations that contribute to the problem. Their analysis of these issues will be informed and provide needed insight. A central aim of the program is to keep stakeholders promptly informed and actively involved in managing smoking-related issues, as they are key participants in the program process itself (WHO, 2012).

Needs assessment is also indispensable in determining the resources and timetable for the program (WHO, 2012). Resources may be limited or unavailable, and designing an effective program requires an evaluation of existing or past smoking cessation programs — identifying what has worked — and adapting successful elements accordingly (WHO, 2012).

Results show that while this type of education and intervention program for smoking cessation may be globally convenient in a web format, it also presents significant barriers that limit the benefits of that format (Huey et al., 1998). The finding that 20% of those who expressed interest in the program require maximum personal and direct interaction and motivation to quit smoking is a strong factor against the effectiveness of this format.
